摘  要:目的 探讨胰岛移植早期血糖控制对大鼠胰岛移植效果的影响。方法 体外实验 :将新鲜制备的SD大鼠胰岛在 4种不同糖浓度中培养 5d后恢复至常规糖浓度中培养 3d ,观察其形态、存活时间及功能变化。胰岛移植实验 :将糖尿病SD大鼠随机分为 3组 :移植及胰岛素控血糖组 ;单纯移植组和未移植 (对照 )组。监测各组的血糖水平。结果 在体外 ,短期的高糖培养对SD大鼠胰岛的形态、存活时间无明显影响 ,但可降低其对高糖刺激的反应能力。在糖浓度恢复正常后 ,该功能可恢复。移植实验中 ,血糖控制组在移植后 14d时 ,受体鼠血糖 ( 2 7.0± 4.0 )mmol/L与对照组 ( 3 3 .1± 1.8)mmol/L间差异有显著性 (P <0 .0 5 ) ,而单纯移植组与对照组的血糖差异仅维持到移植后 8d。结论 移植早期的血糖控制有改善胰岛移植效果的作用。Objective To investigate the influence of early-stage plasma glucose control on the outcome of rat islet transplantation.Methods After isolation and purification,islets of SD rats were cultured in medium with different glucose concentrations for 5 days,followed by another 3-day culture in an ordinary glucose level medium.Morphology,survival time and function of islets were observed.In allogeneic islet transplantation research,streptozotocin-diabetic SD rats were divided randomly into 3 groups:group 1 (Transplanted + Plasma glucose controlled),group 2 (Transplanted + Plasma glucose uncontrolled),and group 3 (without transplantation).Plasma glucose was monitored.Results In vitro,short-term exposure of SD islets to elevated glucose levels showed no detrimental effect on the survival time or morphology of cultured islets,whereas diminishing islet response to an acute glucose stimulus.In transplantation study,plasma glucose of rats in group 1 (27.0±4.0) mmol/L remained different from that in group 3 (33.1±1.8) mmol/L (P<0.05) 14 days after transplantation with the difference being statistically significant,while plasma glucose difference between group 2 and 1 was not of statistical significance 8 days after transplantation.Conclusion Early-stage plasma glucose control improves the outcome of rat islet transplantation.
