基 金:原发性高血压患者动态血压昼夜节律异常与早期肾损害相关性研究 (合同号 :P2 0 0 2 1 1 )
摘 要:目的 :探讨动态血压与尿微球蛋白和原发性高血压 (EH)肾损害的关系。方法 :2 4 h动态血压昼夜节律消失 (A组 )、节律正常 (B组 )各 30例 EH和 30例正常人 (对照组 )的血、尿 β2 微球蛋白 (β2 - MG) ,并对两组参数进行相关性分析。结果 :两组临床资料 ,偶测血压 (CBP) ,日间血压无显著差异 ,而血、尿 β2 - MG均较对照组高 (P<0 .0 0 1 )。尿 β2 - MG在早期时即出现异常 ;A组的 2 4小时血压、夜间血压、血压负荷及尿 β2 - MG均高于 B组 (P<0 .0 1 )。ABPM昼夜节律消失者 ,肾功能损害明显 ,而CBP不能揭示两组病人之间的这种差异。结论 :ABPM和尿 β2 - MG能更好地反映肾损害的关系 。Objective:To explore the relation between ambulatory blood pressure(ABP),the urine microglobulim(MG)and renal damage Methods:ABP monitoring was performed and β 2 MG in urine and blood were determined in 60 essential hypertension (EH) patients and 30 normal people(the comparing group).The rhythm of ABPM with EH patients was disappeared for group A and the other ones was normal for group B Results:It was not different in day blood pressure(DBP) for CBP between 2 groups.But β 2 MG in blood and urine in EH group were higher than that in the comparing group( P <0 001).β 2 MG in urine was abnormal in early with EH patients.The load value in 24h BP and night BP of group A was higher than that of the group B.It was more obvious in the renal damage of the group B ( P <0 01).But CBP can't post the different between the two group Conclusion:ABPM and β 2 MG in urine can reflet the relation between EH and the renal damage.So it is more beneficial to find the earlier renal damage
