摘  要:目的 :探索神经嵌压及断伤与面神经微循环障碍的关系。方法 :用激光多普勒血流计 (LaserDopplerFlowmeter ,LDF)测量 2 5只新西兰大白兔水平段骨管内的面神经血流 ,暴露面神经第二膝部鞘膜 ,施加压迫、切断等处理 ,观察其近侧即面神经水平段血流的变化。结果 :以 10 g重力压迫面神经第二膝部鞘膜 10s可造成近侧神经微循环障碍 ,压迫解除后血流恢复 ;第二膝部切断后面神经水平段LDF值由 787.4± 2 0 9.8mV下降到 3 96.9± 12 2 .8mV(t =12 .91,P <0 .0 1) ,LDF基线平均下降幅度为 5 0 %± 8%。结论 :第二膝部面神经受压或断伤会引起近侧骨管内面神经微循环障碍 ,LDF对研究面神经骨管内嵌压有重要价值 ,提示临床处理面神经第二膝部创伤时 。Objective: To study the microcirculation disorder of the facial nerve entrapment and transection.Method: Laser doppler flowmeter was used in the blood flow measurement of the horizontal segment of the facial canal in 25 new zealand white rabbits.Pressing and section on the second genu of facial nerve were used to find whether it could change the blood flow of the horizontal segment of the facial nerve.Results: LDF of the horizontal segment of the facial nerve was can be changed by the manipulations in the distal nerve segment.10 g gravity was given on the nerve sheath and last 10 s made LDF temporary decreased.The LDF of horizontal facial canal lower 50%±8%(from 787.4±209.8 mV to 396.9±122.8 mV, t=12.91,P<0.01 ).When the second genu were transect.Conclusion: Pressing and section on the second genu of facial nerve can induce the microcirculation disorder of the proximal horizontal segment of the facial nerve.LDF is very useful in the study of facial nerve entrapment.The results suggest that the horizontal segment of the facial nerve should be included in the range of decompression surgery when we handle the nerve trauma of the second genu.
