摘 要:目的 分析通过肱动脉段流速曲线变化推断上肢动脉近端闭塞性病变的可行性。方法 30例受检者 6 0条上肢 ,经数字减影血管造影 (DSA)证实 2 9条上肢动脉近端存在显著狭窄。超声分别以肱动脉段收缩期峰值流速值(PSV)、收缩期加速度值 (AC)、肱动脉段流速曲线形态异常为标准预测上肢动脉近端病变。结果 以肱动脉段AC值≤ 4m/s2 为标准 ,预测上肢动脉近端显著狭窄的敏感性、特异性、准确性分别为 93% ,10 0 % ,97%。以肱动脉段单峰流速曲线出现来预测上肢动脉近端显著狭窄 ,敏感性为 79% ,但特异性达到 10 0 %。结论 分析肱动脉段流速曲线形态和定量参数 ,可推断上肢动脉近端的显著狭窄或闭塞。Objective To study the possibility of predicting occlusive disease in proximal artery of upper extremities through analyzing changes of Doppler waveforms from brachial artery.Methods Thirty patients with 60 upper extremities were included in this study. Significant stenosis in proximal arterial segments of 29 extremities was confirmed by digital subtraction angiography (DSA). The stenosis in proximal upper extremity was predicted by values of peak systolic velocity (PSV), systolic acceleration (AC),and the abnormal waveforms from brachial artery.Results Taking the value of AC ≤4m/s 2 as criteria, color Doppler flow imaging achieved 93% sensitivity, 100% specificity, and 97% accuracy in predicting proximal stenosis. Taking the monophasic Doppler waveform occurred in the brachial artery as criteria, its sensitivity was 79%, but the specificity of the modality was as high as 100%.Conclusion We can predict the moderate to severe stenosis in proximal upper extremity by analyzing the changes of Doppler waveform or hemodynamic parameters from brachial artery.
