摘  要:目的 探讨影响骨微损伤的相关指标和间接判断微损伤的方法。方法 83只SD大鼠,分为3组:去卵巢组、17β-雌二醇替代组及假手术组,分别在3,15及21周时进行骨密度、骨计量学、骨生物力学和生化指标的定量观测,同时应用自制的疲劳损伤试验机(专利号:ZL00 2 25310.0)进行疲劳损伤后观测微损伤,将微损伤的指标分别与上述各项指标进行相关分析,并找出相关指标进行多元线性逐步回归分析,确定有意义的自变量。结果微损伤与生化指标无相关;与松质骨含量较多的胫骨上端部位骨密度相关:与椎骨弹性模量、椎骨最大应力存在负相关关系,并与多个骨形态计量学指标(骨小梁间隔、骨小梁数目、骨小梁厚度、标记百分率和骨形成率)相关,但影响微损伤数目与长度的因素有所不同。结论骨微损伤与松质骨关系密切,受骨微结构影响较大,但判断微损伤的无创方法有待进一步研究。Objective To find out the variables related to bone microdamage and an indirect diagnostic method for the injury. Methods Eighty-three female SD rats were randomly divided into three groups: Group A, ovariectomized (OVX); group B, sham-operated(SHAM) ; and group C, treated with 17 β-estradiol (EST). The measurements of biomechanic parameters, bone mineral density, bone histomorphymetry and determination of biochemical markers were performed at 3,15 and 21 weeks after OVX. After fatigue damage using a home-made bone fatigue damage testing machine (China patent No. ZL00 2 25310.0) , the bone microdamages were measured. Correlation analysis was made between the parameters of microdamage and biomechanics, bone mineral density, bone histomorphymetry, and biochemical markers, respectively. Multiple linear regression analysis was performed between the microdamage parameters and the related independent variables of biomechanics, bone mineral density, and bone histomorphymetry. Results No correlation was found between the microdamage parameters and biochemical markers. There was a correlation between microdamage and proximal tibia containing cancellous bone. There was a negative correlation between microdamage parameters and the elasticity modules and the maximum stress of vertebrae. A significant correlation was found between the parameters of microdamage and of bone histo-morphometry such as trabecular separation (Tb.Sp) , trabecular number (Tb.N), trabecular thickness (Tb.Th), percent labeled perimeter (%Pm) and bone formation rate(BFR) . However, the factors influencing the number and length of microdamage were different. Conclusions The bone microdamage has a close relationship with cancellous bone and bone microstructure. Non-invasive diagnostic methods for bone microdamage need further research .
