摘 要:目的 研究新生儿CD4^+T细胞在细菌感染时的作用及作为临床应用感染性证据的可能性。方法应用流式细胞仪和单克隆抗体,分别测定19例正常足月新生儿和17例细菌感染足月新生儿治疗前后其CD4^+T细胞表面的白介素-2受体α链(CD25)、β链(CD122)、γ链(CD132)和CD154的表达情况。结果 CD25、CD122、CD132和CD154在正常新生儿CD4^+T细胞上的表达较低,分别为(3.84±3.61)%、(0.74±0.53)%、(12.70±8.54)%和(1.08±1.04)%,其在细菌感染组新生儿CD4^+T细胞上的表达分别为(9.89±9.66)%、(5.79±11.14)%、(21.78±11.34)%和(6.69±7.87)%,较正常新生儿组明显升高(P<0.05),且治疗前后均无显著变化(P>0.05)。结论 正常新生儿CD4^+T细胞的CD25、CD122、CD132和CD154表达较低,在细菌感染时表达上调,可作为新生儿细菌感染诊断的参考指标。Objective To investigate the expressions of surface markers on CD4+ T lymphocyte and the possibility of these being used as diagnostic evidences for neonatal bacterial infection. Methods Cordblood samples from 19 healthy terms as controls and peripheral blood samples from 17 terms at the age of ( 14.43±7.91)d with bacterial infection were collected for analyzing CD25, CD122 , CD132 of IL -2R and CD154 by whole blood method with flow cytometry (FCM). Results The lower levels of expression of α chain [ CD25: ( 3. 84±3. 61 ) % ] , (3 chain [ CD122: ( 0. 74±0. 53 ) % ] ,βchain [CD132: ( 12.70±8.54% ) ] of IL-2R and CD154[ (1. 08±1. 04) %] on lymphocytes were noticed in the normal samples. Meanwhile the up - regulation of CD25 [ (9. 89±9. 66)% ] , CD122 [ (5. 79±11. 14)% ]and CD132 [ (21. 78±11. 34) % ] of IL -2R and CD154[ (6.69±7.87)% ] on CD4+ T cell was observed in 17 neonates in their early stage of infection, significantly higher than that in controls (P all< 0. 05). No significant changes in the expression of the surface markers on lymphocytes were found in infected samples after anti - infection therapies. Conclusion The Up - expressions of CD25, CD122, CD132 of IL -2R and CD154 on lymphocytes in infected neonates suggest that IL -2R and CD154 expression on CD4+ T lymphocytes may be used as diagnostic evidences for neonatal bacterial infection.
