摘　　要：目的 观察生脉注射液对慢性心力衰竭(CHF)患者血清内源性洋地黄物质(EDLS)水平的影响,以探讨生脉注射液对地戈辛血药浓度和药动学参数影响的可能途径。方法 将患者40例随机分为甲组(予西医常规治疗)和乙、丙、丁组(在西医常规治疗基础上分别予生脉注射液20、40、60ml静滴),每组10例,观察各组治疗前后患者EDLS水平的变化。结果 生脉注射液在改善心功能和升高EDLS水平方面有量-效关系;其不同剂量组间疗效差异无显著性,但有呈现量-效关系的趋势;EDLS水平与心功能分级呈负相关。结论 生脉注射液可能具有刺激CHF患者体内EDLS分泌和释放的作用,其机制可能与其改善CHF患者的心功能有关。Objective: To evaluate the effect of Shengmai Injection on the serum level of endogenous digitalis-like substance(EDLS) in patients with congestive heart failure (CHF), and to reveal the possible mechanism of the influence on Shengmai Injection on the blood concentration and pharmacokinetic parameters of digoxin. Methods: 40 in - patients, coinciding with the diagnostic standard of CHF, were divided randomly into four groups: the three trial groups (5% glucose solution 200ml added with Shengmai Injection 20ml, 40ml or 60ml, iv drip, n = 10 in each group) and the controlled group(5% glucose solution 200ml, iv drip, qd × 7d, n = 10). The serum level of EDLS in the patients were examined by radio -immunity assay (RIA) before the medication and 7 days later. Results: (1) There were no significant difference in the serum level of EDLS between the low - dosage group (20ml) of Shengmai Injection and the controlled group. (2) The serum level of EDLS in other two treatment groups (40ml, 60ml) were higher than the one of controlled group. Conclusion: (1) The research suggests that Shengmai Injection may increase the secretion and release of EDLS in patients with CHF. (2) It might be one of the mechanisms that Shengmai Injection could improve the heart function of patients with CHF.
