二维超声及彩色多普勒在睾丸扭转诊断中的应用

摘　　要：目的：研究睾丸扭转的二维超声及彩色多普勒图像特点，以便早期诊断并与其他阴囊急诊作鉴别。方法：首先用二维超声显示睾丸、附睾形态结构及内部回声，再用彩色多普勒观察血供状况。将结果与手术病理对照或进行临床治疗随访。结果：７６例阴囊急诊中诊断睾丸扭转２４例，其二维超声及彩色多普勒特征如下：①睾丸及附睾在阴囊内位置的改变，②睾丸内血流明显减少或消失，③睾丸合并附睾弥漫性肿大。结论：彩超诊断睾丸扭转具有较高的准确性、敏感性和特异性，能较准确地判断睾丸扭转的程度，可作为手术复位疗效评价的依据。Objective:To study the image characteristics of two dimensional ultrasonography and color doppler of the testis torsion and to differentiate testis torsion from other scrotal emergency.Methods:The morphologic changes of testis and internal echo were displayed with two dimentional ultrasound, and the blood perfusion in testis was observed with color doppler. The results were compared with those of operation and pathology or clinical follow up.Results:The characteristics of sonography of testis torsion were as follows:①The position changes of testis and epididymis in scrotum.②The clear reduction or disappearing of blood supply in testis.③The whole enlargements of testis and epididymis.Conclusion:The color doppler ultrasonography has high accuracy, sensitivity and specificity, and it is a non traumatic method and can judge the degree of torsion and offer proof for judging efficacy of operational result. It should be regarded as a first means for diagnosing torsion of testis.
