重症法洛四联症患儿围手术期的护理

摘　　要：目的　探讨重症法洛四联症患儿围手术期的护理措施 ,以期提高手术成功率。方法　选择1999年 3月～ 2 0 0 2年 12月我科收治重症法洛四联症患儿 90例 ,在体外循环深低温停循环或深低温低流量下行法洛四联症根治术或姑息术 ,手术前后实施预防缺氧发作、低心排护理、呼吸道及伤口护理等围手术期护理措施。结果　 90例手术患儿中手术成功 84例 ,死亡 6例。术后发生低心排32例 ,胸腔引流偏多 5例 ,经治疗后缓解。随访 3个月～ 3年 ,患儿均恢复良好。结论　重症法洛四联症患儿术前预防缺氧发作 ,术后做好各项护理措施 ,可降低手术后并发症发生 。Objective To discuss the perioperative nursing care for children with severe tetralogy of Fallot so as to improve the operative success. Method From 1999.3 to 2002.12 ninty children patients with severe Tetralogy of Fallot had undergone radical or palliative operation with cardiopulmonary bypass of deep hypothermia and cardiac arrest or deep hypothermia and low flow. Perioperative measures included prevention of hypoxic spells, nursing care of respiratory tract, intra cardiac metric line, chest drainage and incision. Result Among 90 patients with severe tetralogy of Fallot, 84 were saved and 6 were lost. Thirty two cases had postoperative low eardiac output and 5 cases increased amount of chest drainage. All were relieved after treatment, All patients recovered well after follolo up of 3 months to 3 years. Conclusion Preopeative nursing care for prevention of hypoxic spells, and postoperative nursing care including that of low cardiac output, respiratory tract, intra cardiac metric line, chest drainage and incisionare of great importance to decrease postoperative complications and improve the success rate of the operation.
