复合微孔多聚糖止血粉在肋骨骨折内固定术中止血效果的临床分析

摘　　要：目的观察复合微孔多聚糖止血粉在肋骨骨折内固定手术中出血的止血效果并探讨其止血机制。方法对2015年12月至2016年12月116例肋骨骨折内固定手术患者平均分成两组进行临床研究,研究组术中使用复合微孔多聚糖止血粉局部止血,对照组使用纱布压迫、电灼止血等方法止血。观察两组手术时间;出血量;术后第1 d、第2 d、第3 d时引流量;拔管时间。结果两组患者手术时间分别(33.61±10.46)min和(50.61±11.46)min;创面出血量分别为(25.41±15.39)m L和(64.32±16.41)m L;术后第1 d时胸腔引流量为(170.58±26.24)m L和(250.97±28.18)m L,第2 d时为(110.51±15.24)m L和(153.78±18.23)m L,第3 d时为(60.25±15.75)m L和(100.42±16.81)m L;拔管时间分别为(2.12±1.41)d和(4.22±2.01)d;以上结果两组差异有统计学意义(P<0.05)。结论复合微孔多聚糖止血粉应用于肋骨骨折内固定手术中是减少术中出血及减少术后并发症的有效手段。Objective to observe hemostatic effect of composite microporous polysaccharide hemostatic powder in internal fixation of rib fracture and explore its hemostatic mechanism. Methods divide 116 cases rib fracture internal fixation patients from December 2015 to December 2016 into two groups for clinical study. Study group was treated with local hemostasis by composite microporous polysaccharide hemostatic powder, and control group with gauze compression and electrocautery hemostasis. Observe operative time, bleeding amount, drainage volume at 1 d, 2 d and 3 d and extubation time of two groups. Results operation time of two groups was(33.61±10.46) min and(50.61±11.46) min, and wound bleeding volume was(25.41±15.39) m L and(64.32±16.41) m L respectively. Thoracic cavity flow rate 1 d after operation was(170.58±26.24) m L and(250.97±28.18) m L, and that after 2 d was(110.51±15.24) m L and(153.78±18.23)m L, that after 3 d was(60.25±15.75) m L and(100.42±16.81)m L, extubation time was(2.12±1.41) days and(4.22±2.01) days;difference of above results was statistically significant between two groups(P < 0.05). Conclusion compound microporous polysaccharide hemostatic powder is an effective method to reduce intraoperative bleeding and postoperative complications in treatment of internal fixation of rib fractures.
