摘  要:颅脑创伤(Traumatic Brain Injury, TBI)是一个严重的公共卫生问题,可能会对个体造成破坏性的后果。TBI后垂体功能减退在过去的十年里受到越来越多的关注。中重型TBI后患者机体发生一系列的变化,除急性应激反应外,还伴有神情淡漠、沟通障碍、机能减退等一系列神经精神症状。这在过去一直被归因于一种模糊的"震荡后遗症"。而近年来研究表明,神经内分泌功能障碍特别是垂体功能低下在这些现象中起重要作用。这种TBI后垂体功能减退症已被证明是临床上普遍存在的现象。本文旨在综述中重型颅脑创伤后垂体功能减退的病理生理学机制,并强调这一现象在脑外伤后常见神经行为问题发展中的重要性。Traumatic brain injury(TBI)is a serious public health problem that may have devastating consequences for individuals.Pituitary dysfunction after TBI has attracted more and more attention in the past decade.After moderate and severe TBI,a series of changes have taken place in the body.In addition to acute stress reaction,there are also a series of neuropsychiatric symptoms,such as apathy,communication disorder and hypofunction.This has been attributed in the past to a vague'aftermath of shock'.Recent studies have shown that neuroendocrine dysfunction,especially hypophysis,plays an important role in these phenomena.Hypophysis after TBI(PHTH)has been proved to be a common clinical phenomenon.The aim of this article is to review the pathophysiological mechanism of pituitary hypofunction after moderate and severe traumatic brain injury,and to emphasize the importance of this phenomenon in the development of common neurobehavioral problems after traumatic brain injury.
