超声在疤痕子宫再次妊娠中的应用进展

摘　　要：疤痕子宫再次妊娠子宫切口纤维后弹性差,晚孕期容易出现子宫破裂,大出血等,严重威胁母婴安全。超声可观察子宫下段疤痕处情况,为临床判断提供重要的信息。二维超声可见典型图像羊膜囊膨出宫外,三维超声实现多维度扫查,彩色与能量多普勒技术检查胎盘植入、胚胎着床子宫疤痕具有较高的运用价值。高频彩超能清楚地显示子宫下段前壁的三层结构。实时超声造影技术通过组织灌注中微泡反射成像,与组织反射成像有明显差异,对超声远场状况的呈现也优于二维超声与彩色多普勒检查,但超声造影剂对胎儿的影响仍缺乏统一定论。For repeated pregnancy of scar uterine,the fiber near uterine incision is of poor elasticity.Therefore,uterine rupture,hemorrhage and other conditions are prone to occur in the third trimester of pregnancy,which seriously threaten maternal and infant safety.Ultrasound can observe the condition of scar in the lower uterine segment and provide important information for clinical judgment.Typical images of amniotic sac protruding out of the uterus can be observed by two-dimensional ultrasound.Multi-dimensional scanning can be achieved by three-dimensional ultrasound.Color and energy Doppler technology has high application value in examination of placenta implantation and embryo implantation in uterine scar.High frequency color Doppler ultrasound can clearly show the three-layer structure of the anterior wall of the lower uterine segment.Real-time contrast-enhanced ultrasound imaging through microbubble reflection imaging in tissue perfusion is obviously different from tissue reflection imaging and it is also better than two-dimensional ultrasound and color Doppler examination in the presentation of the far field of ultrasound.However,there is still a lack of unified conclusion on the effects of ultrasound contrast agent on the fetus.
