摘　　要：目的　观察连续静 -静脉血液滤过 (continuousVenovenoushemofiltration ,CVVH)治疗难治性充血性心力衰竭 (CHF)疗效以及安全性。方法　行股静脉插管术 ,置入双腔导管 ,建立临时性血管通路 ,选用YT -12 0SPAN血滤器进行床旁CVVH。结果　 4 6例CHF患者经此方法治疗 ,全身浮肿明显消退 ,肺部湿罗音消失 ,呼吸困难明显缓解 ,由端坐位或半卧位转成平卧位。在血流速率 15 0 - 180ml/min、超滤量平均达 12 .387L时 ,患者均未出现低血压等并发症 ,平均动脉压波动幅度 (114± 3.86 )mmHg ,心率波动幅度 (14± 4 .1)次 /分。 结论　CVVH可以安全地用于治疗CHF及其并发的高度水肿。Objective to watch continuous venovenous hemofiltration (CVVH) and reach the curative effect and safety of the treatment of refractory congestive heart failure (CHF).Methods To carry on beside continous venovenous hemofiltration using YT-120 SPAN blood filter by taking femorao vein encheiresis , inserting double channel catheters and setting up critical blood vessel passages.Results After this treatment,46 cases of the patients of CHF has been improved , their anasarca disappeared , the lung moist rates cleared up , the asthma obviously remissed , and they can breath from orthopnea or semireclining position to horizontao position . When the blood flow rate is at 150-180 ml/min, and the average utlratiltration rate at 12.387L , these is no appearance of hypotension or complication , with the result of the average arterialpressure flutuation 11+3.86mmHg and heart rate 14+4.1/min . Conclusion CVVH can be safely used for treatment of the congestive failure and its complications such as hign edema , the electrolyte disturbance and acid-base imbalance.
