胎儿颈部软组织厚度产前超声筛查的价值

摘　　要：目的 :探讨超声测量胎儿颈部软组织厚度 (Nuchal Skinfold Thickness,NT)在检出染色体异常疾病 2 1-三体综合征中的价值。方法 :经腹超声测量 32 0 3例中晚孕期胎儿颈部软组织厚度 ,并进行统计分析及与染色体核型分析作对照。结果 :在 32 0 3例胎儿中 ,NT增厚者有 16 2例 ,其中 4例为 2 1-三体 ,假阳性为 15 8例 ,假阴性 2例 ,其敏感性为 6 6 .6 7% ,特异性为95 .0 6 % ,准确性为 95 %。结论 :超声测量胎儿 NT值 ,对 2 1-三体胎儿的筛查有重要的参考价值。不失为一种简便、安全、可重复、非侵入性、孕妇易于接受的新方法。Objective:To study the value of the Nuchal Skinfold Thickness (NT) of fetus in diagnosis of fetal 21-trisomy syndrome.Methods:NT of 3 203 fetuses from middle to late gestation were measured with ultrasonography and the results were statistically analyzed. NT was compared with the karyotype analysis.Result:162 fetuses had the thicker NT among the 3 203 fetuses,and 4 of them were 21-trisomy syndrome. The false positivity and false negative were 158 and 2 respectively.The sensitivity,specficity and accuracy of ultrasonography were 66.67%,95.06% and 95%.Conclusion:The ultrasonographic measurement data of NT have an important reference value in prenatal screening of fetal 21-trisomy syndrome.It is a new method of convenience, safety, repeat ability and nonintrusion. It is welcome by pregnant women.
