摘  要:目的 探讨新生儿缺氧缺血性脑病 (HIE)时血浆内皮素 (ET)、头颅CT以及新生儿行为神经测定(NBNA)三项检测在不同时期的变化及其相互关系 ,评估此三项指标单一及相结合后对临床的指导意义。方法 放射免疫法检测HIE患者不同时期的血浆ET值 ,同时进行头颅CT及NBNA检测。结果 ET值的升高与临床中重度病情关系密切。而升高的持续时间可能与预后关系较大 ;头颅CT在早期与临床的符合率不足 70 % ,因而早期CT正常不能排除HIE ;NBNA测定结果则与HIE的临床经过基本一致 ,但其缺点是易受到新生儿状态的影响。结论 三种指标相结合可以更为有利于客观地判断病情 ,指导治疗和预后 ,进而有利于开展早期干预 。Objective To investigate the change and interrelation of the plasma endothelin (ET),intracranial computed tomography (CT) and neonatal behavioral neurological assessment (NBNA) in different periods of neonatal hypoxic ischemic encephalopathy (HIE);evaluate the clinically guiding significance of the above three items in single and combined modes.Methods Plasma ET of HIE baby patient in different period was detected,and at the same time to get the data of CT and NBNA.Results There was a close relation between the increase of ET and clinical condition.And,the duration of ET increase was related to the prognosis;the positive result of intracranial CT in early time was coincident with the clinical signs in less than 70% of the cases.So normal CT in early time does not deny HIE.NBNA score is identical with clinical signs,but it may be affected by the baby′s conditions.Conclusion To combine ET determination,CT and NBNA together is a valuable method to judge the state of an illness objectively,guiding the treatment and prognosis,benefiting early protecting the baby from becoming disabled.
