冠心病血清可溶性粘附分子水平与血脂异常的关系

摘　　要：目的 :通过检测冠心病 (CHD)患者血清可溶性细胞间粘附分子 - 1(sICAM - 1)、血管细胞间粘附分子 - 1(sVCAM -1)和E -选择素 (sE -selectin)水平 ,分析血脂水平与可溶性粘附分子水平 (CAMs)的相关性 ,以期探讨血脂异常对CAMs表达的影响。方法 :用ELISA法检测了 6 0例经冠脉造影诊断的CHD患者血清slCAM - 1、sVCAM -l、sE -selectin水平 ,并以 18例健康体检者作为对照。结果 :与对照组比较 ,sICAM - 1水平于稳定性心绞痛 (SAP)组增高 ,有显著性意义 (P<0 0 5 ) ,于不稳定性心绞痛 (UAP)组、急性心肌梗塞 (AMl)组增高更为显著 (P <O 0 1) ;sVCAM - 1、sE -selectin水平于UAP组、AMI组增高有显著性意义 (P <O 0 1) ,SAP组增高不明显 (P >0 0 5 )。在CHD组内 ,血脂异常组与血脂正常组相比 ,sICAM - 1水平于血脂异常组增高 ,有显著性意义 (P <O 0 5 ) ,sVCAM - 1在两组间也有显著性差异 (P <0 0 5 )。多元回归分析表明 ,TC、LDL -C是sVCAM - 1的独立影响因素 ,LDL -C、HDL -C是sICAM - 1、sE -selectin的独立影响因素。结论 :CHD患者可溶性CAMs水平增高 ,血脂水平影响血清可溶性CAMs水平。Objetives: To explore the relationship of soluble cell adhesion molecules with dyslipidemia in coronary heart disease Methods:By using ELISA method,the levels of sICAM-1,sVCAM-1 and sE-selectin in serum were measured for 60 patients with coronary heart disease diagnosed by coronary arteriography and for 18 healthy persons as controls Results: The level of slCAM-1 with CHD was significantly higher than that of controls,especialy in UAP and AMI groups(P<0 01) The UAP and AMI group had higher level of sVCAM-1 and sE-selectin than that in control group(P<0 01) CHD patients with dyslipidemia had higher level of slCAM-1(P<0 01) and sVCAM-1(P<0 05)than those patients with normal lipid Multivariable analysis showed that TC and LDL-C were independently associated with sVCAM-1 while LDL-C and HDL-C were independent factors affecting the levels of slCAM-1 and sE-selectin Conclusions:The level of soluble CAMs was elevated in patients with CHD and dyslipidemia affected the level of soluble CAMs
