摘  要:目的 了解 β 受体阻断剂是否可以预防或减少老年下肢骨折患者心血管的应激反应。 方法 对 2 0 0 0年 1月至 2 0 0 2年 8月因非暴力髋部骨折住院的 3 88例老年病人进行了回顾性研究。根据心功能将其分为四组。心功能Ⅳ级和Ⅲ级组给予比索洛尔 ,心功能Ⅱ级及以下者随机分为 2组 ,一组给予比索洛尔 ,一组不予此类药品 ,了解服药后各组中心血管相关症状及某些心律失常的发生率。结果 入院时心功能Ⅲ级以上组无论是心血管相关症状发作还是有症状的心律失常发作均多于心功能Ⅱ级或以下组 ,服药后 2周 ,其心血管相关症状发作及发生的有症状的心律失常与心功能Ⅱ级或以下非服药组相似。服药的心功能Ⅱ级以下组 2周后其心血管相关症状和心律失常的发生少于对照组。结论 β 受体阻断剂可能有益于减少老年病人下肢骨折后心血管的应激反应。Objective To investigate whether the cardiovascular stress could be prevented by β-blocker in the elderly patients with fracture of lower extremity.Methods 388 patients with fracture of lower extremity hospitalized from Jan.2000 to Aug.2002 were investigated retrospectively, and grouped on the basis of classification of their heart function.The patients with class Ⅳ of heart function were grouped into A,class Ⅲ into B,and class Ⅱ or lower into C and D randomly.The group D was as a control and others were administered with bisoprolol.The occurrences of cardiac symptoms or symptomatic arrhythmias were observed in the groups.Results The patients with class Ⅲ or above had a higher rate of attack of cardiac symptoms or symptomatic arrhythmias,as compared with groups C and D when they were just hospitalized,but their symptoms and arrhythmias occurred similarly to those of control group after they were administered with bisoprolol for two weeks.The morbidity was lower in group C two weeks after taking bisoprolol than those of the control group.Conclusion β-blocker may be beneficial to reduce the occurrence of cardiac stress in the elderly patients with fracture of lower extremity.
