摘 要:目的 :研究COX 2和iNOS在宫颈癌发生发展中的作用及其预后价值。方法 :采用免疫组化SP法检测6 4例宫颈癌组织及 31例CIN和 30例正常宫颈组织中COX 2和iNOS的表达水平。结果 :①宫颈癌组织COX 2和i NOS阳性表达率分别为 4 8.4 4 %和 85 .94 % ,明显高于正常宫颈组织 ;②宫颈癌组织COX 2和iNOS的表达水平与临床分期、组织学分型及分化程度无关 (P >0 .0 5 ) ,与高危因素 (淋巴结转移、肿瘤大小、宫旁浸润或脉管浸润 )有关 ;③宫颈癌组织COX 2和iNOS的表达之间未发现相关性 (P >0 .0 5 ) ;④宫颈癌组织COX 2阳性表达与无瘤生存时间缩短和总生存率降低有关。多因素分析表明 ,COX 2阳性表达是宫颈癌患者复发的一个显著预后因素。结论 :宫颈癌组织中COX 2和iNOS表达水平明显上调与宫颈癌的发生发展有关 ;宫颈癌患者预后不良与COX 2表达有关 ,而与iNOS表达无关 ,免疫组化检测COXObjective: To ascertain the possible roles of COX 2 and iNOS in the tumorigenesis and development of uterine cervical carcinoma, and to evalulate their impact on the prognosis of cervical cancer. Methods: Immunohistochemical assay was used to detect COX 2 and iNOS in all specimens, including 64 cervical cancers, 31 cervical intraepithelial neoplasia (CIN) and 30 normal cervices. Results: ① The overall positive expression rate of COX 2 and iNOS in cervical cancer was 48.44% and 85.94% respectively, higher than that in normal cervix; ② The intensity of expression of COX 2 and iNOS in cervical cancers was not related to FIGO stage, histologic type and tumor differentiation(P>0.05). Positive COX 2 and iNOS was related to high risk factors(lymph node metastasis, large tumor size, parametrial invasion or lymphvascular space involvement); ③ The expression of COX 2 was not correlated with that of iNOS in cervical cancers(P>0.05); ④ Positive COX 2 was associated with decreased disease free survival and a lower overall survival. On multivariate analysis,positive COX 2 expression was a significant prognostic variable for recurrence(P<0.01). Conclusion: The present results suggest that COX 2 and iNOS may play vital roles in the tumorigenesis and development of the cancer via different ways.Unfavorable prognosis were associated with COX 2 expression while not associated with iNOS. Immunohistochemical assessment of COX 2 might be a useful molecular marker for predicting prognosis in cervical cancers.
