摘　　要：目的评价下腔静脉滤器置入后经ATD消融及尿激酶溶栓治疗下肢深静脉血栓形成的应用价值。方法22例经彩色Doppler超声检查证实为下肢深静脉血栓患者,先于下腔静脉内放置滤器,后经尿激酶溶栓治疗,其中6例同时行月国静脉ATD消融。尿激酶用量为25～100万U/天,平均给药5～10天。结果22只下腔静脉滤器全部释放到位,展开良好。6例经ATD消融患者,下肢深静脉即时开通。经尿激酶溶栓治疗后痊愈7例,显效12例,有效3例,无效0例。无一例出现肺动脉栓塞及颅内等重要脏器出血。结论下腔静脉滤器置入后经ATD消融及尿激酶溶栓治疗下肢深静脉血栓形成是安全、有效的。Objective To evaluate the clinical effectiven ess of thrombosis amplification and thrombolysis for treating lower extremity deep venous thrombosis(LEDVT)after placement of inferior vena cava filter(IVCF).Methods Twenty -two patients proved LEDVT by color Doppl er ultrasound underwent preventive IVCF placement for thrombosis ampli fication and thrombolysis by urokinase.The dose of urokinase was 250000～1000000U /d.Results A total of 22IVCF were successfully deployed without disposition and mi gration.The therapeutic effect were divided into four scales after thro mbolysis by urokinase:fully recover from LEDVT(n =7);remarkable recovery(n =12);effective treatment (n =3);ineffective treatment (n =0).No pulmonary embolism and hemorrage oc curred during the procedure of throm bolysis.Conclusion Thrombosis amplification and thrombolysis for treating LEDVT is s afe and effective after preventive p lacement of IVCF.[
