老年睡眠呼吸暂停综合征与多器官疾病关系的研究

摘　　要：目的　 研究睡眠呼吸暂停综合征 (SAS)与多器官疾病的关系。　 方法　对 3 2 86例老年人进行了SAS与多器官疾病调查 ,并随机对 976例进行了多导睡眠监测仪监测。　 结果　 3 2 86例中有SAS 95 9人 (2 9% )。SAS组临床表现 :白天嗜睡、遗尿、头痛、夜间多尿、记忆力减退、性欲减退、乏力、不安腿、头晕、胸痛、反应迟钝、语言障碍、精神异常、憋醒和惊恐均较对照组显著增多 (P <0 0 5～ 0 0 1)。SAS组高血压、冠心病、脑卒中、心肌梗死、心绞痛、高脂血症、糖尿病、心力衰竭、哮喘、呼吸衰竭、肥胖症、痴呆症、夜尿症和抑郁症明显增多 ,与对照组比较有显著差异 (P <0 0 5～ 0 0 1)。 10年中病死率SAS组为 86例 (8 9% )与对照组 42例 (1 8% )比较有显著差异 (P <0 0 1)。　 结论　SAS组多器官疾病增多 ,机制可能与SAS引起低氧血症、高碳酸血症、高脂血症、高粘滞血症等有关。Objective To research the relationship between sleep apnea syndrome(SAS) and multiple organ disorders(MOD). Methods The study investigated 3 286 elderly cases of SAS and MOD,and 976 cases were examined by polysomography randomly. Results Among 3 286 elderly cases,959 cases(29%)suffered from SAS.The clinical features of day time lethargy,enuresis,headache,night time polyuria,dysmnesia,hyposexuality,asthenia,restless leg,dizziness,thoracalgia,diminished response,aphasia,psychic abnormality,awakening and frightened in SAS group were statistically significant compared with control group( P <0 05～0 01).The complications of essential hypertension,coronary heart disease,stroke,myocardial infarction,angina pectoris,hyperlipidemia,diabetes mellitus,heart failure,nocturnal asthma,respiratory failure,obesity,dementia,nocturia and depression in SAS group were statistically significant compared with control group( P <0 05～0 01).The SAS group mortality in 86 cases(8 9%) versus control group in 42 caseas(1 8%) were statisitically significant( P <0 05～0 01) in ten years. Conclusions It is suggested that the incidence of MOD in SAS group is higher.The mechanism may be due to hypoxaemia,hypercapnia,hyperlipidemia and hyperthrombinemia.
