特发性中枢性性早熟女孩血清瘦素水平及临床意义

摘　　要：目的 :探讨瘦素在特发性中枢性性早熟女孩 (ICPP)发病中的作用。方法 :用放射免疫法测定31例ICPP女孩血清瘦素并与30例同年龄健康女孩比较。结果 :ICPP组体重和血清瘦素明显高于对照组 ,分别为 (24 8±5 7)kg、(20 0±5 0)kg,(7 9±3 8)μg/L和 (4 9±1 2) μg/L ;ICPP中乳房发育TannerⅢ期组血清瘦素高于Tanner期Ⅱ组 ,分别为 (10 6±6 2) μg/L和 (7 0±2 1) μg/L(P<0 05) ;ICPP组和对照组血清瘦素与体重、体重指数正相关 ,r分别为0 6633、0 4989、0 7720、0 8130(P均<0 01)。结论 :青春期启动需要有一定的体重和体脂的积累 ,瘦素作为脂肪细胞合成和分泌的一种激素可能参与了青春期启动和性的发育。Objective:To explore the effect of leptin in pathogenesis of girls with idiopathic central precocious puberty(ICPP).Methods:The clinical analysis involved31cases of girls with ICPP and30normal girls,and the lepin levels of serum of them were examined by radioimˉmunoassay.Results:The weight and lepin of ICPP girls were obviously high compared with nomal,which were(24.8±5.7)kg,(20.0±5.0)kg and(7.9±3.8)μg/L,(7.0±2.1)μg/L respectively(P<0.01);According to Tanner stage of breat development in ICPP,the leptin level of serum in Tanner stageⅢwas higher than that of Tanner stageⅡ,which were(10.6±6.2)μg/L,(7.0±2.1)μg/L respectively(P<0.05);there was positive correlation between leptin level of serum and weight,body mass index(BMI)in ICPP and nomal girls,r=0.6633,0.4989,0.7720,0.8130respectively(all P<0.01).Conclusion:The leptin may participate in pathogenesis in girls with ICPP as the hormone produced and secreted by fat cell.
