斑秃的中医辨证与情绪测量分析

摘　　要：目的对斑秃患者进行中医辨证并评估其情绪状态。方法用中医辨证方法对 65例斑秃患者进行辨证分型 ,同时采用中医肝脏象情绪量表进行情绪测量分析 ,并设 3 0例健康人对照组。结果 65例斑秃患者中肝郁气滞证占 2 6.2 % ,肝肾阴虚证和血虚生风证各占 3 3 .8% ,血瘀毛窍证占 6.2 % ;76.9%的患者存在不同程度的情绪状态异常 ;肝郁气滞和血虚生风证斑秃患者的肝经因子分、焦虑因子分、抑郁因子分及总分均显著高于健康人 (P <0 .0 5 ) ,肝肾阴虚证患者除抑郁因子分外 ,其它评分亦均明显高于健康人 (P <0 .0 5 ) ;斑秃患者各个因子分、总分及情绪状态异常率均无性别差异 ;斑秃 (AA)组与全秃 +普秃 (AT +AU)组间评分差异也无显著性意义。结论斑秃患者的中医辨证多为肝证 ,多数患者存在不同程度的焦虑和抑郁 ,临床上可用中医肝脏象情绪量表对斑秃患者的情绪状态进行评估。Objective To investigate the correlation of the syndrome differentiation with the mood status evaluation among patients with alopecia areata(AA).Methods This study was carried out among 65 cases with AA,with a controlling group including 30 healthy persons.They were all identified in their patterns on the basis of syndrome differentiation and evaluated on their mood status based on the emotion rating scale (ERS) for the Liver visceral manifestation(LVM).Results Among the cases with AA,the pattern of Liver Qi depression (LQD) accounted for 26.2%,Liver-Kidney Yin deficiency(LKYD) and blood deficiency engendering wind (BDEW) each accounted for 33.8%,and blood stasis in hair portal(BSHP) for 6.2 %.Furthermore,76.9% of cases were seen with various degrees of abnormal mood.The scores of Liver channel,anxiety factor and depression factor were significantly higher among cases of AA with LQD and BDEW than that of healthy persons(P<0.05).Moreover, the scores of all the factors were also significantly higher among the cases of AA with LKYD than that of heahty people,except that of depression factor.All the individual and total scores of these factors were seen with no differences in their sex distribution among AA cases.These were also true in the evaluation of scores between AA group and the group of AT plus AU.Conclusions AA cases are most frequently seen with Liver patterns based on syndrome differentiation and with various degrees of anxious and depressive mood.Their mood status can be evaluated on the basis of ERS.
