摘  要:目的 探讨心肌声学密度测定以正常心肌组织细微变化的分辨能力。方法 对41名健康医学生胸骨旁左室腱索水平短轴观的前壁、室间隔、下壁及侧壁的心肌进行声学密度(AD)检测,比较吸入硝酸异山梨醇酯(ISDN)前后测值的变化。结果 ①心脏各壁的AD平均值(AII)之间的差异均有显著性,以下壁测量值最大,前壁次之,室间隔最小,与多位作者的研究结果互不相同。②吸药前后心脏前壁及室间隔AII的差异有显著性,其中吸药后前壁的AII变小,而室间隔则相反,AII变大。心脏下壁、侧壁的AII以及各壁的AD最大值与最小值之差(PPI)、离散度(SDI)在吸药前后的差异均无显著性。结论 ①按目前的仪器条件,不同的操作者对心脏各壁的AD测定无法在统一条件下进行,故AD技术对心脏的研究应在相同频率、增益(包括总增益及TGC、LGC)和深度条件下,对同一切面同一部位心肌的不同状态进行比较才合理。②AD技术对正常人左室腱索水平短轴观的前壁及室间隔心肌组织的改变分辨力较高,敏感定量指标为AII。Objective To evaluate the myocardium acoustic densitometry (AD) in detecting minute variations of normal myocardium tissue. Methods AD parameters of the interventrivular septum, anterior, inferior and lateral walls of the left ventricle were assessed at parasternal short-axis view on chordae tendineac level before and after inhaling isosorbide dinitrate (ISDN) in 41 healthy medical students to evaluate the ability of AD in detecting minute variations of normal myocardial tissue. Results ①The differences of average image intensity (AII) for-various walls are significant. The AII values of inferior wall, anterior wall and interventricular septum were in descending order. which was different from results from previous literatures. ② The AII values of anterior wall and interventricular septum were significantly different before and after inhaling ISDN. The post-ISDN AII value decreased in anterior wall and increased in interventricular septum. Neither AII values of the inferior and lateral walls, the peak-to peak intensity (PPI), nor standard of image intensity (SDI) in each wall showed difference before and after inhalation. Conclusion ① There is no standard AD parameters due to the limitation of the present technology, The AD parameters are comparable only if they are assessed under the same frequency, gain (including total gain, TGC and LGC), and depth. ② The AD technique is highly sensitive in evaluation of myocardium of anterior wall and interventricular septum. And AII is a sensitive quantitative index.
