摘  要:目的 探讨治疗高龄病人复杂的股骨转子间骨折的一种内固定方法。方法  2 0 0 0年 4月至 2 0 0 3年 1月 ,采用Link股骨近端解剖型钢板 ,治疗平均年龄为 77 4岁的高龄病人股骨转子间骨折 3 6例 ,其中男 2 1例 ,女 15例 ,左侧2 0例 ,右侧 16例。按照Tronzo分型 ,Ⅲ型 2 3例 ,Ⅳ型 13例。每例病人均有 1~ 3种合并症 ,伤前生活均可自理。结果 术前住院 3~ 8天 ,平均 6 7天。住院时间 15~ 2 8天 ,平均 2 2 8天。无 1例死亡 ,无髋内翻及明显肢体短缩 ,1例加压螺钉松动。随访时间平均 2 5个月 ,12例超过 3年 ,均无死亡 ,2 6例病人可弃拐行走 ,恢复伤前生活质量。结论 对高龄病人复杂的股骨转子间骨折 ,积极处理合并症 ,早期选择可靠有效的内固定方法 ,能降低病死率和致残率 ,取得良好临床治疗效果。Objective To discuss an interfixation of complicated intertrochanferic fracture in advanced patients.Methods From Apr.2000 to Jun.2003,36 patients with femur intertrochanteric fracture were operated on using Link MAY anatomical proximal proximal femur bone plate.In all 36 cases,the average age was 7.4 years,21 cases were male and 15 cases female.20 cases were on the left,and 16 on the right.According to the Tronzo classification,there were 23 cases of thpe Ⅲ fracture,13 cases of type IV fracture.Every patient had 1~3 kinds of complications.All the patients could take care of themselves before injury.Results The average time before operation was 6.7 days(ranging from 3 to 8 days).No patient died.No poor result was fonnd.Complicaion was loosening of spongiosa screws in l case.There was no hip varus or shortening of limb.The fixation was reliable and effective.All the patients were followed up for 25 months in average,12 cases were followed up over 3 years.No patient died.26 patients could walk by themselves.Conclusion For femur intertrochanteric fracture in advanced patients,the death rate and disable rate can be reduced and satisfactory result can be obtained by giving active therapy to the complications and using reliable and suitable interfixations.
