肾功能RIA在评价老年高血压病肾功能变化中的价值

摘　　要：目的:探讨肾功能放射免疫分析在评价老年高血压病肾功能变化中的价值。方法:采用放射免疫分析(RIA)测定36例老年高血压病患者及30例健康老年人sβ_2-MG、uβ_2-MG、uAlb、uIgG,并测定血清BUN和Gr。结果:老年高血压病组sβ_2-MG、uβ_2-MG、uAlb、uIgG测定值均高于对照组(P<0.05～0.01),Ⅰ期老年高血压患者uβ_2-MG、uAlb已有明显增高(P<0.05～0.01);Ⅱ期uβ_2-MG、uAlb、uIgG增高(P<0.05～0.01);Ⅲ期sβ_2-MG、uβ_2-MG、uAlb、uIgG均显著增高(P<0.01)。高血压病组血清BUN及Cr与时照组比较无统计学意义(P>0.05)。结论:老年高血压病在Ⅰ期时已有肾小球、肾小管的损害,随着病情的进展,肾小球、肾小管功能损害加重。肾功能RIA是检测老年高血压患者早期肾功能损害的灵敏的实验方法。Objective:To investigate the value of renal functions radioimmunoassay (RIA)in evaluation of renal functions lesion among elderly essential hypertension(EH) . Methods:The levels of serum β2-MG(sβ2-MG) , urine β2-MG(uβ2-MG),urine albumin (uAlb) and urine IgG (uIgG) were measured in thirty-six elderly EH and thirty healthy elderly controls with RIA.Serum BUN and Cr were also assayed with biochemistry methods. Results: Generally, the levels of sβ2-MG,uβ2-MG,uAlb,uIgG in the elderly EH were higher than the control group (P <0.05 -0.01) . The levels of uβ2-MG and uAlb were obviously increased in stage I eldrly EH ( P < 0.05 - 0.01); Besides uβ2-MG, uAlb, uIgG were increased in stage II elderly EH ( P < 0.05- 0.01). All of the RIA indexes (sβ2-MG,uβ2-MG,uAlb, uIgG) were significantly increased in stage III elderly EH (P < 0.01) .Comparatively, serum BUN and Cr were not significantly different between the two groups (P >0.05). Conclusion:The renal glomerulus and tubules functions have been damaged in stage I elderly EH. Following the development of EH, the renal glomerulus and tubules functions become worse. It is shown that the renal functions RIA is a sensitive experimental method for evaluating early renal function lesion in patients with elderly EH.
