摘 要:目的对泡型包虫病重流行区宁夏西吉县的中间宿主啮齿类动物和人群进行调查,了解该地泡型包虫病中间宿主的分布情况及人群的感染情况,为卫生行政部门制定泡型包虫病的防治措施提供科学依据。方法啮齿类动物的调查采用夹捕法,人群流行病学的调查采用问卷填写,人群血清包虫抗体检查采用酶联免疫吸附试验(ELISA)。所有的信息输入到Excel中并进行统计分析。结果共捕获啮齿类动物10种133只。捕获最多的啮齿类动物为长尾仓鼠87只,占65.4%。调查1772人中泡型包虫病血清学检查阳性者共有119人,血清阳性率为6.72%。不同性别间(χ2=12.59,P=0.00)、不同区域间(χ2=8.71,P=0.033)血清阳性率的差异有统计学意义,总体来看不同年龄段、回、汉民族间泡型包虫病血清阳性率的差异均无统计学意义(P均>0.05)。女性随年龄的增加血清阳性率明显升高。结论西吉县的儿童青少年仍处在包虫病的高危环境中,发病的潜在危险还在增加,应多方面采取防治措施。Objective To identify the driver of the transmission dynamics of Echinococcus multilocularis( E.m.) through the intermediate host population( rodent species) and the indicator of the egg- exposure through serological response amongst children and teenagers at local level. Methods The rodents investigation was undertaken by trapping. The population surveys were carried out amongst school children and teenagers. The specific antibody against E. m. antigens were detected by ELISA assay. All the data was input Excel form.Results A total of 133 rodents were captured in 10 species. The dominant species was captured as cricetulus longicaudatus,accounting for 65. 4%( 87 /133). Serologic positive rate appeared at 6. 7% in 119 individuals among 1772 children / teenagers. The comparisons occurred significantly between gender( χ2= 12. 59,P <0. 05) with higher positive in females than in males,and between different regions( χ2= 8. 71,P < 0. 05);but there were not significant differences in age groups( χ2= 5. 89,P > 0. 05),and in ethnic groups( χ2=1. 25,P > 0. 05). The distribution of domain susceptibility rodent species for E. m. life- cycle showed a highly correlative with the location of the high sero- positive rate amongst children. Conclusion The high sero- positive rate amongst school children indicates that the E. m transmission is now still active in Xiji County. In near future measurements should be taken into account for the control policy development immediately.
