摘  要:目的探讨线粒体通透性转运孔(m PTP)是否参与脂肪乳救治布比卡因心脏毒性时对能量代谢的影响。方法 H9C2心肌细胞复苏培养,随机将其分为4组(n=3):布比卡因组(BPV)加入1mmol·L-1BPV;布比卡因+脂肪乳组(BPV+LE)同时加入1mmol·L-1BPV和1%LE;布比卡因+脂肪乳+环孢素A组(BPV+LE+Cs A)加入1mmol·L-1BPV、1%LE以及m PTP抑制剂Cs A(1μmol·L-1);布比卡因+脂肪乳+苍术苷组(BPV+LE+Atr)加入1mmol·L-1BPV、1%LE以及m PTP开放剂Atr(20μmol·L-1)。4组分别孵育24h后,用高效液相色谱仪检测其能量代谢(ATP、ADP、AMP),Hoechst33342/PI法检测细胞的凋亡率。结果与BPV组比较,BPV+LE组和BPV+LE+Cs A组ATP、ADP、AMP含量增加,细胞凋亡率下降(P<0.05),BPV+LE+Atr组ATP含量增加(P<0.05)而ADP、AMP的含量以及细胞凋亡率差异无统计学意义(P>0.05);与BPV+LE组比较,BPV+LE+Cs A组的ATP、ADP、AMP的含量增高、细胞凋亡率降低(P<0.05),BPV+LE+Atr组ATP、ADP、AMP含量降低,细胞凋亡率增加(P<0.05);与BPV+LE+Cs A组比较,BPV+LE+Atr组ATP、ADP、AMP含量均降低,细胞凋亡率增加(P<0.05)。结论脂肪乳通过抑制m PTP的开放逆转布比卡因所致H9C2心肌细胞毒性时能量生成的减少以及细胞凋亡率的增加。Objective To investigate whether mitochondrial permeability transition pore( m PTP) was involved in the impact of lipid emulsion on energy metabolism in bupivacaine- induced H9C2 cardiac cytotoxicity.Methods After recoveried and cultured,H9C2 cardiomyocytes were seeded in 96- well plates at the density of 1 × 105 cells / m L,then randomly divided into 4 groups( n = 3 each) : Bupivacaine grou( BPV),cells were cultured with medium containing 1mmol · L- 1BPV. Bupivacaine + lipid emulsion group( BPV + LE),cells were cultured in medium which added 1mmol·L- 1BPV and 1% LE. Bupivacaine + lipid emulsion + Cyclosporin A group( BPV + LE + Cs A),cells were cultured with medium containing 1mmol · L- 1BPV,1% LE and 1μmol·L- 1Cs A( m PTP inhibitor). Bupivacaine + lipid emulsion + Atractyloside group( BPV + LE +Atr),cells were cultured in medium with 1mmol·L- 1BPV,1% LE and 20 μmol·L- 1Atr( m PTP opener).After 24 hours of incubation,the energy metabolism( ATP,ADP,AMP content) were measured by high- performance liquid chromatography( HPLC) and apoptotic rate were calculated by Hoechst33342 / PI. Results Compared with group BPV,the contents of ATP,ADP,AMP were significantly increased and apoptotic rate were significantly decreased in BPV + LE and BPV + LE + Cs A group( P < 0. 05) while the contents of ATP were significantly increased( P < 0. 05). Compared with group BPV + LE,the contents of ATP,ADP,AMP were increased and apoptotic rate was decreased in BPV + LE + Cs A group( P < 0. 05) while in BPV + LE +Atr group ATP,ADP,AMP contents were significantly lower and apoptotic rate was higher significantly( P <0. 05). The contents of ATP,ADP,AMP were significantly lower and apoptotic rate was significantly higher in BPV + LE + Atr group than those in BPV + LE + Cs A group( P < 0. 05). Conclusion The lipid emulsion reversed the reduction of energy production and the increase of apoptosis induced by bupivacaine through suppressing m PTP opening.
