血清表皮生长因子水平与药物流产后阴道出血的相关关系

摘　　要：目的:探讨血清表皮生长因子(EGF)水平与药物流产后阴道出血时间的关系,及流产后加服米非司酮(50 mg)或米索前列醇(200 μg)对阴道出血时间的影响。方法:选取90例临床诊断为宫内早孕,要求药物流产的妇女,随机分为3组,采用盲法分别给与安慰剂、米非司酮(50 mg)或米索前列醇(600μg),并测定流产前血清EGF水平。每组再随机抽取10名孕妇,分别测定流产后血hCG水平,分析EGF、hCG与药流后阴道出血天数的关系。结果:血清EGF水平与流产后阴道出血时间成正相关,血清EGF与hCG水平有一定相关性(r=0.428)。流产后加服米非司酮50 mg或米索前列醇200μg,均能明显缩短阴道出血时间(P<0.05)。结论:血清EGF水平影响流产后阴道出血时间,当流产前血清EGF水平达到或超过2.6 μg/dL时,流产后立即加服米非司酮(50 mg)或米索前列醇(200 μg)可有效缩短阴道出血时间。Objective:To explore the relationship between the level of serum epithelial growth factor(EGF) and vaginal bleeding days after medi- cal abortion, and the effect of extra administration of mifeprostone or misoprofil on the EGF, hCG and vaginal bleeding days. Methods; A number of 90 women who were clinically diagnosed as early pregnancy and had no physical complication and inhibition of mifeprostone or misoprofil and wanted to terminate pregnancy by medicine, were randomly devided into 3 groups, who were respectively administered placebo, mifeprostone (50 mg) or misoprofil (200 μg) after routine medical abortion. Before first administration, serum EGF and hCG were assessed. Then 10 women were randomly choosen from each group, and their serum hCG were assessed after abortion. The relationship between EGF with HCG and vaginal bleeding days was analyzed. Results;A positive relationship was found between the serum EGF and vaginal bleeding days after abortion, EGF related to hCG (before abortion) (the relative coefficient is 0.428). Whatever was extra administrated, either 50 mg mifeprostone or 200 μg misoprofil, the vaginal bleeding days could be shortened significantly all the same (P<0. 05). Conclusion: The level of serum EGF affect vaginal bleeding days, especially when the level of serum EGF is 2. 6 μg/dL, if extra 50 mg mifeprostone or 200 μg misoprofil was administered immediately after abortion, the vaginal bleeding days would be shortened significantly.
