呼吸道变应性疾病临床特征及吸入性变应原分布

摘　　要：目的分析广州地区变应性呼吸道疾病的临床特征及常见吸入性变应原分布情况。方法收集变应性鼻炎和/或支气管哮喘患者122例,分成为鼻炎组、哮喘并鼻炎组和哮喘组,行血清吸入性变应原Ig E检测及皮肤点刺试验,比较其临床特征及吸入性变应原分布情况。结果哮喘并鼻炎组血清总Ig E水平明显高于鼻炎组及哮喘组(P值均<0.01);哮喘并鼻炎组血清嗜酸粒细胞水平明显高于哮喘组(P=0.012);哮喘组螨变应原阳性比例明显低于鼻炎组和哮喘并鼻炎组(P值均<0.01);哮喘组树花变应原阳性比例明显低于鼻炎组和哮喘并鼻炎组(P=0.011;P=0.001)。结论螨、树花粉及霉菌为诱发变应性呼吸道疾病的常见吸入变应原,支气管哮喘为异质性疾病,部分哮喘发病与变应性气道炎症相关。Objective Objective: To analyze the clinic features of allergic respiratory disease and the distribution of inhalant allergens in Guangzhou. Methods 122 allergic rhinitis and/or asthma patients were divided 3 groups: the rhinitis group,the asthma combined with rhinitis group,the asthma group. All the patients had taken the serum inhalation allergens Ig E test and/or the skin prick test. The clinical data and the distribution of inhalant allergens were analyzed in 3groups according to the test. Results The serum total Ig E level of asthma and rhinitis group is significantly higher than that of rhinitis group and asthma group( P<0. 01 all). The count of eosinophils of asthma and rhinitis group is significantly higher than that of the asthma group( P = 0. 012). The ratio of mite allergen positive in asthma group is significantly lower than that of remain 2 groups( P < 0. 01 all). The ratio of tree pollen allergen positive in asthma group is significantly lower than that of the rhinitis group and the asthma combined with rhinitis group( P = 0. 011; P = 0. 001). Conclusion Mites,tree pollen and mold were common inhalation allergens to induce allergic respiratory diseases. Bronchial asthma is a heterogeneous disease,part of the pathogenesis of asthma is associated with allergic airway inflammation.
