摘　　要：目的探讨'冬病夏治'穴位敷贴联合针刺治疗结合西医常规疗法治疗慢性阻塞性肺疾病(COPD)稳定期合并焦虑抑郁的疗效。方法将符合入选标准的57例COPD患者随机分为两组,观察组29例,对照组28例。对照组给予西医常规疗法治疗,观察组在对照组基础上加用中医针灸治疗(包括穴位敷贴及针刺)。评估患者中医证候、生活质量评分、汉密顿焦虑量表(Hamilton Anxiety Scale,HAMA)及汉密顿抑郁量表(Hamilton Depression Scale,HAMD)积分。结果观察组中医证候量化评分及生活质量评分均高于对照组,差异有统计学意义(P <0.05);观察组在改善呼吸困难程度上低于对照组(P <0.01);与对照组比较,观察组焦虑抑郁量表积分更低,差异有统计学意义(P <0.01)。两组治疗期间均无明显不良反应发生。结论穴位敷贴联合针刺治疗能够改善COPD稳定期患者的咳痰喘症状,可以优化患者焦虑抑郁状态并提高生活质量。Objective To investigate the efficacy of acupoint application combined with acupuncture combined with Western medicine in the treatment of chronic obstructive pulmonary disease(COPD)with stable anxiety and depression.Methods A total of 57 patients with COPD who met the inclusion criteria were randomly divided into two groups,29 in the observation group and 28 in the control group.The control group was treated with conventional western medicine,and the observation group was treated with traditional Chinese medicine acupuncture(including acupoint application and acupuncture)on the basis of the control group.Patients were assessed for TCM syndrome,quality of life score,Hamilton Anxiety Scale(HAMA),and Hamilton Depression Scale(HAMD)scores.Results The scores of TCM syndromes and quality of life scores in the observation group were higher than those in the control group,the difference was statistically significant(P<0.05).The observation group was lower than the control group in improving the dyspnea(P<0.01);In comparison,the scores of the anxiety and depression scales in the observation group were statistically significant(P<0.01).No significant adverse reactions occurred during the treatment period.Conclusion Acupoint application combined with acupuncture can improve the symptoms of cough and asthma in patients with stable COPD,and can optimize the anxiety and depression of patients and improve their quality of life.
