肘关节隐匿性损伤的影像诊断

摘　　要：目的探讨肘关节损伤后X线、CT检查阴性患者的MR表现。方法随机抽取57例肘关节外伤患者,X线检查骨质结构阴性并行MR检查的患者进行回顾性分析。结果 57例肘关节外伤患者中,22例X线检查骨质及软组织阴性,MR显示肘关节周围软组织损伤;18例X线检查骨质及软组织阴性,MR显示肘关节腔积液,肘关节周围软组织损伤,肱骨掌侧或鹰嘴窝脂肪垫显影移位征阳性;17例X线检查骨质阴性,X线显示肘关节掌侧或鹰嘴窝脂肪垫显影移位征阳性,MR显示肘关节周围软组织损伤,肘关节腔积液,肱骨掌侧或鹰嘴窝脂肪垫显影移位征阳性,骨质结构信号异常。结论 MR可以较好的显示肘关节髓质骨及周围软组织的情况,避免了肘关节骨挫伤及软组织损伤漏诊,对于指导临床及时治疗具有重要意义。Objective To investigate MR performance after elbow injury with negative findings examined by X-ray.Methods 57 patients with elbow injury examined by X-ray and MR were randomly enrolled and analyzed retrospectively.Results Among these patients,22 cases with negative findings by X-ray examination showed soft tissue injury around the elbow with MR;18cases with negative findings in bone and soft tissue by X-ray examination showed elbow joint cavity effusion,soft tissue injury around the elbow,humerus volar or shift of the olecranon fossa a positive fat pad sign by MR;17cases presented with negative findings in bone tissue by X-ray examination,humerus volar or shift of the olecranon fossa was a positive fat pad sign by X-ray.Soft tissue injury around the elbow,joint cavity effusion,humerus volar or shift of the olecranon fossa a positive fat pad sign,abnormal signal of bone structure were shown by MR.Conclusion MR is superior to displaying the medullary bone and soft tissue around elbow,avoiding the misdiagnosis of elbow bone contusion and soft tissue injury,which is very important to guide the treatment.
