成人风心病瓣膜置换术诱导期间罗库溴铵药效学的评价

摘　　要：目的 :评价成人风心病瓣膜置换术诱导期间罗库溴铵的临床肌松效应。方法 :选择 2 0例风心病瓣膜置换术的患者 ,利用肌松监测仪监测等效剂量的罗库溴铵 (0 6mg/kg)或维库溴铵 (0 1mg/kg)的起效时间和临床有效作用时间 ,并评价患者的插管条件。结果 :罗库溴铵的起效时间快于维库溴铵 (92± 2 6秒 / 14 8± 6 7秒 ) ,临床有效作用时间无明显差异 (39± 11 2 / 4 3± 6 7分 )。两组患者的插管条件好 ,且组间无差别。结论 :在风心病患者行瓣膜置换术的诱导期间 ,0 6mg/kg罗库溴铵具有起效快 ,插管条件好 ,中时效的临床肌松效应 。Objective:To evaluate the pharmacological effect of rocuronium in patients with rheumatic heart disease(RHD)during induction of anesthesia.Methods:Twenty patients with RHD undergoing valve replacement surgery participated in this double blinded study,and were distributed into two groups(n=10 cases)randomly.With monitoring of train-of-four trials(Org.Comp)and bispectral index(Space-laboratory medic inc.),patients were induced with 0.05-0.1mg/kg midazolam,10-15μg/kg fentanyl and equivalent dosage of 0.6mg/kg rocuronium or 0.1mg/kg vecuronium(Org.Comp)as a initial rapid intravenous bolus.The value of TOF was obtained,also the condition of intubation was evaluated.Results:Rocuronium had a faster onset time than vecuronium(92±26sec/148±6.7sec),but the same clinical effective time(39±11.2min/43±6.7min)with vecuronium.Both drugs could provide good intubated conditions,and there was no difference between the two groups.Conclusion:In cardiac valve replacement of patients with rheumatic heart disease,rocuronium should be used for rapid-sequence anesthesia induction.
