Blood Pressure, Arterial Compliance and Intima-media Thickness in Carotid and Radial Artery in Normotensive and Hypertensive Subjects

摘  要:目的 分析血压、动脉顺应性及颈动脉、桡动脉内膜 -中层厚度 (intima -mediathickness ;IMT)之间关系。方法 采用血管外超声及动脉脉搏分析仪分别测量 81例血压正常者及 3 3例高血压患者的颈动脉、桡动脉IMT及动脉弹性指数C1、C2。结果 高血压组较血压正常组C1、C2降低 ,颈动脉内膜 -中层厚度 (CIMT ;carotidIMT)、颈动脉内径(carotiddiameter;CD)增加 ,两组桡动脉内膜 -中层厚度 (RIMT)及桡动脉内径 (radialdiameter ;RD)相比无显著差异。血压正常组中无高血压家族史者较有高血压家族史者C1较高而收缩压 (SBP)、脉压 (PP)、CIMT较低。相关分析 :C1、C2与年龄、SBP、PP呈负相关 ;CIMT与年龄、SBP、舒张压 (DBP)、PP呈正相关 ;RIMT与SBP、DBP呈正相关。调整年龄、性别、BMI、CD、RD、及血生化指标后 ,C1、C2与SBP、DBP、PP呈负相关 ;CIMT与SBP、PP呈正相关 ,与DBP无显著相关性。而RIMT与血压相关无显著性。结论 CIMT及C1。Objective To investigate the relationshi p of blood pressure, arterial compliance and carotid intima-media thickness (CIMT)and radial intima-media thickness(RIMT). Methods Eighty-one normotensives and 33 hypertensives were recruited in this study. Arterial compliance (C1-large artery and C2-small artery) were measured by pulse wave analysis and CIMT and RIMT by B-mode-ultrasound. Result s Compared with normotensives, C1 and C2 were decreased in hypertensive s, while CIMT and carotid diameter(CD) were increased. No difference of RIMT and radial diameter(RD) was shown between two groups. In normotensive group, C1 of the subjects without family history of hypertension(FHH) was higher than that of the subjects with FHHbut systolic blood pressure(SBP), pulse pressure(PP), CIMT, of the subjects without FHH were lower. Correlation analysis showed that C1 and C2 were inversively correlated with age ,SBP and PP. CIMT was positively correlated with age, SBP, diastolic blood pressure(DBP) and PP. RIMT was positively correlated with SBP and DBP. After adjustment for age, sex, body mass index(BMI), CD, RD, C1 and C2 were inversively correlated with SBP, DBP and PP. CIMT was positively correlated with SBP and PP. No correlation between RIMT and blood pressure was found. Conclusion C1 , C2 and CIMT measurements are sensitive indicators of evaluating abnormalities of the function and structure of vascular-wall in atherosclerosis and hypertension.
