血小板膜表面GPⅡb/Ⅲa、CD62p、CD63诊断妊高征的临床价值

摘　　要：目的 :检测妊娠高血压综合征患者血小板活化膜表面糖蛋白GPⅡb/Ⅲa、CD6 2p和CD6 3的表达 ,探讨其在妊高征诊断中的临床价值。方法 :采用流式细胞术检测不同程度妊高征患者及正常孕妇血小板表面GPⅡb/Ⅲa、CD6 2p及CD6 3的表达 ,并比较重度妊高征患者治疗前后各项指标的变化。结果 :轻、中、重度妊高征患者血小板表面GPⅡb/Ⅲa、CD6 2p和CD6 3的表达率分别为 (14 .5± 6 .3) %、(11.8± 4 .9) %、(10 .2± 3.9) % ;(2 0 .7± 9.1) %、(15 .6± 5 .8) %、(13.7± 4 .8) % ;(5 0 .4± 2 0 .9) %、(35 .2± 13.6 ) %、(31.7± 10 .3) % ,均显著高于正常孕妇组。重度妊高征患者显著高于轻中度患者 ,治疗后GPⅡb/Ⅲa、CD6 2p和CD6 3的表达率均显著低于治疗前。结论 :妊高征患者血小板明显活化 ,随病情加重血小板活化增强。Objective:To investigate the expression of platelet activation glycoproteins GPⅡb/Ⅲa,CD62p and CD63 and their clinical significance in pregnancy induced hypertension.Methods:Expressions of GPⅡb/Ⅲa,CD62p and CD63 in patient groups and normal gravida group were detected by flow cytometry(FCM).Results:The expression rates of GPⅡb/Ⅲa,CD62p and CD63 in mild,moderate and severe PIH were (14.5±6.3)%,(11.8±4.9)%,(10.2±3.9)%;(20.7±9.1)%,(15.6±5.8)%,(13.7±4.8)%;(50.4±20.9)%,(35.2±13.6)%,(31.7± 10.3)% respectively.All the markers in PIH were significantly higher than those of normal gravida group.The markers in severe PIH were significantly higher than those in mild and moderate PIH,and after treatment expressions of GPⅡb/Ⅲa,CD62p and CD63 declined evidently.Conclusion:Platelet activation may play an important role in PIH and the activation rates of platelets increases as the disease progressed.Antiplatelet activation therapy may be very important for controlling PIH and preventing bad outcome of perinatal stage.
