婴儿维生素K缺乏的临床干预研究

摘　　要：目的　探讨口服维生素 K1 对广西婴儿 VKD的控制效果。方法　 360 7例活产婴随机分为干预组 1 655例 (出生～ 3月龄口服维生素 K1 2 .5mg共 1 0次 ) ,对照组 1 952例 (不用维生素 K1 ) ,观察生后 4个月内两组发生 VKDB情况。对住院分娩的足月新生儿随机抽样分别口服维生素 K1 2 .5mg、5mg及不服药各 60例 ,生后 3天测定 G- 6- PD酶活性及观察黄疸、消化道、皮肤等反应。结果　干预组无 1例发生出血 ,对照组发生 VKDB1 1例 (5.63% ,P <0 .0 1 )。口服维生素 K的足月新生儿对 G- 6- PD酶活性均无影响 (P >0 .0 5) ,无黄疸加重、过敏及胃肠道不良等反应。结论　婴儿小剂量 1 0次口服维生素 K1 预防 VKDB是简便、易行、经济、安全。Objective To study VKD control for the infants taken Vitamin K 1 (VitK 1) orally in Guan gxi.Methods In Guigang City of Guangxi, 3607 live births were randomly divided into interven ed group (given oral dose of VitK 1 2.5mg for 10 times from born to the third m onths) and control group (without any VitK 1), which were observed occurrence o f VKDB during 4 months after the infants born. The term neonates delivered in ho spital were randomly given oral dose of 2.5mg VitK 1, 5mg VitK 1 and without a ny VitK1 respectively for 60 cases per group. On the third day after the infants born, they must be tested G-6-PD enzyme assay, and be observed the reaction o f jaundice, digestive tract, and skin. Results No one bled (0%) among 1655 inf ants in the intervened group. There were 11 cases with VKDB (5.63%, P<0.01) among 1952 infants in the control group. All 128 term neonates given oral dose o f VitK 1 did not have any effect on G-6-PD enzyme activity (P>0.05), and there were also no any reaction of worsen jaundice, allergy and gastrointestinal tract. Conclusions It was a simple, convenient, easy, economic, safe and effective intervened measure that the infants took a small dose of oral VitK 1 for 10 times to prevent VKDB.
