北京某郊区县卫生防疫系统现况调查

摘　　要：目的　了解基层卫生防疫系统的现状 ,为完善卫生防疫体系以应对今后可能的突发公共卫生事件提供依据。方法　采用机构考察、个人访谈、问卷调查等定性与定量研究相结合的方法 ,于 2 0 0 3年 5— 6月 ,对北京市某郊区县疾病控制中心 (CDC)及 17所乡级卫生院的卫生防疫系统现状进行调查。结果　防保科承担本辖区内的儿童保健、计划免疫、精神卫生、传染病防治、流行病学调查、卫生宣教等日常工作 ;防保人员 70 %左右的工资需单位自筹。防保人员占总业务人员比例 5 %及以下的占 4 1.2 % ;各乡卫生院防保人员占服务人口的比例为平原乡平均 1.7人 /万人 ,少或半山乡 1.6人 /万人 ,山区乡 2 .6人 /万人 ;平原乡防保人员年纪轻、学历高、多受过相关专业训练 ,山区乡防保人员年龄偏大、均为中专学历 ,5 7.1%为非预防医学专业毕业、4 2 .9%为非医学专业毕业人员经培训后从事防保工作。结论　该地需要进一步明确区、乡两级预防保健系统的职能 ,工作中急需得到政府部门的理解与配合 ;区、乡两级预防保健系统资金及防保人员不足、专业人才匮乏问题有待解决。Objective To learn the status of primary prevention and health care station, and to make the system of disease prevention and health care perfectly so as to deal with potential outburst incident of public health. Methods The observation, personal in-depth interview and structured questionnaire methods were conducted to survey the status of disease prevention and health care System. From May to June in 2003, center of disease control and 17 village public health centers in a County, Beijing of China were investigated. Results The routine work of preventive and health care section was about children health care, plan immunity, spirit health, prevention and care for infection, epidemic survey, health education et al. The 70 percent salary of staffers was paid by township hospital. About 41.2 percent of township hospital, prevention and health care staffers were less than 5%. The proportions of staffers were 1.7/10000 in plain village, 1.6/10000 in half-montanic village, 2.6/10000 in montanic village. The staffers were young, trained, and with high educational level in plain village, contrarily, in montanic village staffer status were very different, which 57.1% of them graduated from non-public health, 42.9% of them were non-medical basic but just had a little training. Conclusion The function of disease prevention and health care system in township and village should be definitude farther. The shortage of fund and understaffing also should be solved.
