儿童特发性便秘的诊断(86例临床分析)

摘　　要：目的 :探讨儿童特发性便秘的诊断。方法 :对 2 0 0 0年 6月至 2 0 0 2年 1 0月经结肠造影和肛门直肠测压诊断的 86例特发性便秘的临床资料进行分析。结果 :男 4 4例 ,女 4 2例 ,婴儿期 9例 ,幼儿期 6 6例 ,学龄期 1 1例。结肠造影检查 86例 ,其中未见异常 75例 ,直肠扩张 5例 ,乙状结肠冗长 3例 ,直肠扩张合并乙状结肠冗长 3例 ;肛门直肠测压 4 4例 ,其中未见异常1 6例 ,排便弛缓反射异常 1 2例 ,直肠肛门抑制反射 (RAIR)异常 2 2例 ,感觉功能障碍 1 0例。获得随访 6 8例 ,其中痊愈 4 7例 (6 9. 1 % ) ,好转 1 1例 (1 6 . 2 % ) ,无效 1 0例 (1 4 7% )。结论Objective:To discuss the diagnosis of the idiopathic chronic constipation in children.Methods:86 cases'clinical information in barium enema and anorectal manometry were analysed from June,2000 to October,2002.Results:There are 44 males and 42 females,9 cases of whom were in babyhood(28 days to 12 months),66 cases were infants (1 year to 6 years) and 11 cases in school age (7 years to 12 years),86 cases have been examined by barium enema,75 cases of them normal,5 cases rectum dilation,3 cases redundancy of sigmoid colon,3 cases rectum dilation combined with redundancy of sigmoid colon,44 cases have been examined anotectal manometry 16 cases of them were normal,12 cases abnormal in defecation atony reflx,22 cases abnormal in RAIR and 10 cases sensation impediment.68 cases have been follow uped.47 cases (69.1%) recoveried,11 cases (16 2%)improved and 10 cases (14 7%) withowt effect.Conclusion:barium enema combined with anotectal manometry can enhance the diagnosis of the idiopathic chronic constipation in children.
