摘　　要：目的 :探讨抗Sm抗体阳性系统性红斑狼疮 (SLE)的临床特征。方法 :对 2 75例抗Sm抗体阳性SLE患者临床资料进行分析 ,并与 2 90例抗Sm抗体阴性SLE患者临床资料进行比较。结果 :抗Sm抗体阳性SLE患者年龄小、病程短、光敏感、关节炎、口腔溃疡较多见 (P <0 0 5或P <0 0 1) ;血沉快、蛋白尿、补体下降发生率较高 (P <0 0 5或P <0 0 1) ;抗URNP抗体、抗Rib抗体阳性率高 ;抗Sm抗体阳性组与阴性组之间SLEDAI积分差异无显著性 (P >0 0 5 )。结论 :(1)抗Sm抗体阳性SLE患者临床表现明显 ,易被早期诊断。 (2 )抗Sm抗体、抗URNP抗体、抗Rib抗体联合检测 ,可能对SLE诊断意义较大。 (3 )抗Sm抗体水平与疾病活动性无一致关系。Objective To explore clinical features of systemic lupus erythematosus (SLE) with positive anti Sm antibody. Methods The clinical data of 275 cases of SLE with positive anti Sm antibody were analyzed and compared with those of 290 cases of SLE with negative anti Sm antibody. Results The results showed that the course of SLE with positive anti Sm antibody was shorter than that of SLE with negative anti Sm. The age of patients in the former was younger than that in the latter. Photosensitivity, arthralgia and oral cavity ulcer were observed more frequently in SLE patients with positive anti Sm antibody ( P <0 05 or 0 01). Higher erythrocyte sediumentation rate, proteinuria and low levels of C3 had a higher incidence in those of positive anti Sm antibody ( P <0 05 or 0 01). Anti URNP antibody and anti Rib antibody were more than those of patients with negative anti Sm antibody. The intergral of SLEDAI had no significant difference between two groups ( P >0 05). Conclusion (1)The clinical manifestations were obvious and the early diagnosis is easily determined in SLE patients with positive anti Sm antibody. (2) It had a high diagnostic value to SLE that anti Sm antibody, anti URNP antibody and anti Rib antibody were jointly tested. (3) The level of anti Sm antibody wasn’t consistent with the activity of SLE.
