螺旋CT门静脉造影在肝胆胰疾病术前评估中的价值

摘　　要：目的 :探讨螺旋CT门静脉造影 (SCTP)在肝胆胰疾病术前评估中的价值。方法 :对 76例肝胆胰疾病患者进行SCTP检查 ,其中手术治疗 49例 ,以此为对照 ,分别对肝硬化门脉高压症、原发性肝癌、肝门部胆管癌、胰头癌等疾病的SCTP表现逐一分析。结果 :SCTP能完整显示门静脉系统的全貌及与病变之间复杂的空间解剖关系 ,可直观评价门静脉的管径、管壁受侵程度、阻塞程度及侧支循环表现 ,其术前诊断与手术对照的符合率为 10 0 %。结论 :SCTP是门静脉系统全面可靠的检查方法 。Objective To evaluate the value of spiral CT portography (SCTP) before liver or biliary duct or pancreas surgery.Methods SCTP was performed in 76 cases with diseases of liver,biliary duct or pancreas,in which of them 49 cases were treated by surgery.An evaluation of portal hypertension hepatocirrhosis and portal vein involvement by cancers of liver,biliary duct or pancreatic head was contrasted.The finding was compared with the surgical and pathological results respectively.Results SCTP can well display portal vein system entirely as well as complicated anatomy relationship of diseases.The patency of portal vein and presence of collateral in portal hypertension were well revealed.The extent of portal vein involvement was also precisely demonstrated.Conclusion SCTP is a credible imaging modality in evaluating portal vein system before surgery,and it is of instructional value in clinical use.
