摘  要:白介素 18是新的细胞因子 ,能诱导产生γ 干扰素 ,增强自然杀伤细胞及辅助性Th1细胞的活性 ,与Th1细胞分化及细胞毒效应有关。特发性血小板减少性紫癜 (ITP)是自身免疫性出血性疾病 ,其发病与Th细胞之间的失衡有关。为了进一步探讨免疫调控细胞因子在ITP发病中的作用及其临床意义 ,采用ELISA双夹心法 ,检测了32例ITP患者和 18名正常人血浆中Th细胞相关细胞因子 :IL 18、TNF α和sC5b 9的水平变化。检测发现 :ITP患者血浆IL 18、TNF α及sC5b 9水平增高 ,且血浆IL 18升高与TNF α和sC5b 9呈正相关。结论 :血浆IL 18、TNF α、sC5b 9等细胞因子水平异常增高与ITP患者的Th1/Th2失衡相关 ,在ITP患者的免疫紊乱中起一定的作用 。To explore the role of immune regulating cytokines in pathogenesis of the idiopathic thrombocytopenic purpura (ITP) and its clinical significance, the levels of IL 18 , TNF α and Sc5b 9 in plasma of 32 ITP patients and 18 normal individuals were detected using ELISA methods. The results showed that IL 18, TNF αand sC5b 9 levels in plasma of ITP patients were higher than that in normal individuals. The level of IL 18 was positively correlated with the levels of TNF αand sC5b 9. In conclusion, The rising levels of the IL 18, TNF α and sC5b 9 were correlated with disorder of Th1/Th2 subsets, and may contribute to the immune dysfunction in ITP patients. The dynamic observation of these cytokines may be useful in directing the clinical treatment for ITP patients.
