摘 要:目的 探讨耳显微外科手术中神经监护仪使用的意义。方法 全麻下对 31例中耳乳突手术及 1例巨大听神经瘤扩大迷路切除术术中进行面神经监护 ,记录术中不同部位同步、非同步反应波的最大波幅 ,并进行比较 ,及记录诱发同步反应的最小电刺激反应电流阀值。结果 不同记录部位的非同步反应波无显著性差异 ,有面神经鞘膜裸露时电刺激反应电流阀值显著低于无面神经鞘膜裸露 ,电刺激反应电流阀值高于 2 5mA时提示术后面神经功能较差。结论 神经监护仪术中能有效地帮助面神经定位 ,预防损伤面神经 。Objective Study the application on facial nerve monitor in otology micro-surgery. Methods Apply facial nerve monitoring under general anesthesia for 31 eardrum,mastoid process surgeries and 1 huge acoustic neuroma resection,in order to record and compare the ultimate amplitudes of in-phase and non-in-phase in different positions during the surgeries.The minimum electronic stimulate threshold values are also recorded. Results No substantial differences exist in non-in-phase reaction wave in different positions.The reactive electronic threshold value will be significantly lower in the case of facial nerve sheath nudity compared with non-nudity.A threshold value over 2.5mA implies bad function of facial nerve after operation. Conclusion Nerve monitor is helpful in orientating facial nerves during operation,preventing possible damnification of facial nerves and may be an expectation index of nerve function fter surgery.
