显微外科技术治疗周围神经嵌压症(附36例分析)

摘　　要：目的　周围神经嵌压症并非少见 ,大多数患者予以针灸、理疗及局部封闭等非手术治疗可获治愈 ,少数功能障碍明显者需做松解术治疗。方法　 36例患者均行神经外膜松解术或神经束间松解术。结果　平均随访 1 0个月。疗效评定标准为正常、好转 2项。近期有记录其疗效者 2 1例 ,远期随访者 1 3例。在近期有记录疗效 2 1例中 ,有 7例 (正常 3例 ,好转 4例 )未获随诊。获得远期随诊仅有 1 4例 ,由近期好转变为正常者 5例 ,近期正常到远期仍为正常者 4例 ,近期好转到远期仍为好转者 5例。近期未记录 ,远期随诊疗效正常者 7例 ,好转者 4例。结论　神经嵌压症如诊断明确 ,出现运动神经麻痹及感觉神经功能障碍者 ,必须行神经外膜松解术 ,我们认为神经外膜松解与神经束间松解在疗效上差异不大。所以 ,当发现有神经麻痹症状 ,早期探查神经 ,尽快解除神经压迫因素 ,恢复神经内、外血液供应。Objective The peripheral nerve impacted compression is seldom seen in clinic and most of them are treated and cured with acupuncture, physiotherapy and local blockage. A few of them with severe symptom are used the releasing method. Methods The materials of 36 cases of such patient were analyzed and all of them were treated with epineurium lysis or intra-branch lysis. The curative standard were normal and better with 10 months follow-up. Conclusion If the diagnosis of this disease is obtained with nerve dysfunction, the epineurium lysis or intra-brach lysis should performed and these two methods might have same effect. The early operation could release the compression and improve the blood supply.
