摘  要:目的:了解肝大部分切除后再生过程中甲胎蛋白(α-FP)、端粒酶活性(TA)和肝脏组织学、肝功能的变化,探讨促肝细胞生长因子、生长激素治疗对肝细胞再生及甲胎蛋白、端粒酶活性和肝脏组织学、肝功能的影响.方法:选取体重200g 左右大鼠36只并随机分成6组,其中组1进行肝大部分切除后生长激素(GH)治疗;组2进行肝大部分切除后应用肝细胞生长因子(HGF)治疗;组3仅进行肝大部分切除而不给予任何药物治疗;组4、组5、组6均不进行切除手术,但组4和组5分别给予生长激素、肝细胞生长因子治疗,组6则不给予任何治疗.1wk 后取材,再生肝组织端粒酶活性采用 PCR-ELISA 法检测,血清甲胎蛋白含量采用夹心 ELISA 法检测.另外,肝组织还作病理切片观察.结果:手术组 TA,AFP,ALT,GGT 值均显著高于非手术组(两组 TA 分别为1.33±0.26,0.496±0.054,P<0.01;两组 AFP 分别为62.9±13.5μg/L,8.9±2.8μg/L,P<0.01;两组 ALT 分别为1204±0.57 nkat/L,61.11±15.48 nkat/L,P<0.05;两组 GGT 分别为10.72±1.58 nkat/L,5.75±1.78nkat/L,P<0.01),而手术组 ALB 则显著低于非手术组(分别为28.1±3.0 g/L,31.4±2.3 g/L,P<0.01).组1的 ALT显著高于组2和组6(三组 ALT 分别为82.16±18.5 nkat/L,60.83±7.96 nkat/L,66.67±11.02 nkat/L,P 分别<0.01,0.05),组2和组3的 ALB 显著低于组6(三组 ALB 分别为28.3±4.1 g/L,26.9±2.3 g/L,31.8±2.1 g/L,P 分别<0.05,0.01).组1、组2和组3的 TA(三组 TA 分别为1.56±0.18, 1.38±0.10,1.04±0.12)及 AFP 值(三组 AFP 值分别为75.9±9.6μg/L,59.6±12.2μg/L,53.1±7.0μg/L)显著高于组4、组5及组6(三组 TA 分别为0.48±0.04,0.52±0.06,0.48±0.06)(三组 AFP 值分别为10.3±3.4μg/L,9.6±2.4μg/L,6.9±0.8μg/L)(两指标 P 值均<0.01).组织病理学研究发现手术组的残肝和非手术组的肝脏大体组织形态学差异不大,而手术组的再生肝则无典型的肝小叶结构,但可见肝窦,且细胞排列较残肝和非手术组肝脏更密集.结论:促肝AIM:To study the effect of growth hormone(GH)and hepatocyte growth factor(HGF)on rat liver regeneration after partial hepatectomy(PH). METHODS:A total of 36 rats were divided randomly into 6 groups.Group 1 was treated with growth hormone after PH;Group 2 was treated with hepatocyte growth factor after PH;Group 3 was not treated with any drug after PH; Groups 4 and 5 without PH were only treated with GH and HGF respectively,and Group 6 was designed as the normal control without PH and drugs treatment.After 7 days treatment,the sera and liver tissues of all rats were collected.The serum AFP was detected by ELISA and the telomerase activity(TA)of liver tissue was determined by PCR-ELISA.Meanwhile,the liver was studied by pathology. RESULTS:The levels of TA,AFP,ALT and GGT in the all hepatectomized groups were significantly higher than those in the groups without PH:The levels of TA of the two groups were 1.33+0.26 and 0.496+0.054 respectively(P<0.01); the levels of AFP of the two groups were 62.87+13.50 μg/L and 8.93+2.76 μg/L respectively(P<0.01);the levels of ALT of the two groups were 1204±257 nkat/L and 61.11± 15.48 nkat/L,respectively(P<0.05);and the levels of GGT of the two groups were 10.72+1.58 nkat/L and 5.75±1.78 nkat/L respectively(P<0.01).But the level of ALB was lower than these groups(28.1±3.0g/L,31.4±2.3g/L,P<0.01).The level of ALT in Group 1 was higher than Group 2(82.16±18.5 g/L, 61.11±15.48 g/L,P<0.01).And the levels of ALB in Groups 2 and 3 are significantly lower than that in Group 6(28.2± 4.1 g/L,26.9±2.3 g/L,31.9±2.1 g/L,P<0.05,0.01 respectively).The levels of AFP and TA in Groups 1,2 and 3 were higher than those in Group 4,5 and 6(AFP of the six groups were 75.9±9.6 lag/L,59.6±12.2 μg/L,53.1±7.0 μg/L, 10.3±3.4 μg/L,9.6±2.4 μg/L and 6.9±0.8 μg/L respectively, TA of the six groups were 1.56±0.18,1.38±0.10,1.04±0.12, 0.48±0.04,0.52±0.06 and 0.48±0.06 respectively P<0.01).
