彩色多普勒超声对产前胎儿脐带绕颈的诊断

摘　　要：目的确定彩色多普勒超声对诊断产前胎儿脐带绕颈的应用价值。方法432例年龄为20岁～35岁的孕妇于产前1周经彩色多普勒超声检查,以探测是否存在胎儿脐带绕颈情况。结果432例产妇中,产前胎儿脐带绕颈发生率为22.69%(98/432),超声阳性预测值和阴性预测值分别为97.96%(96/98)和99.40%(334/336),准确率为99.54%(430/432)。在脐带绕颈胎儿中,绕颈1周、2周及3周者分别为74、19和5例。彩色多普勒超声与血流显像直接证实,胎儿脐带绕颈的声像特征是随着脐带绕颈的不同周数而异。结论彩色多普勒超声与血流显像可为准确诊断产前胎儿脐带绕颈存在与否提供非常可靠的信息。Objective To determine the usefulness of color Doppler ultrasonography for diagnosing the prenatal fetal umbilical cord round the neck.Methods 432 pregnant women aged 20～ 35 years old underwent the color Dopler ultrasonograhy of abdomen at one week before child birth to monitor whether presence of umbilical cord entanglement.Results Among 432 puerperants,the prevalence rate of fetal umbilical cord round the neck was 22.69 % (98/432).Positive predictive value and negative predictive value were 97.76 % (96/98)and 99.40 % (334/336) respectively.The rate of accuracy was 99.54 % (430/432).Among the foetus with umbilical cord round the neck, one circle,two circles and three circles of entanglement were found in 74,19 and 5 cases respectively.Color Doppler ultrasonography and color Doppler flow imaging directly demonstrated that the characteristic findings of fetal umbilical cord round the neck were varied with different circles of entanglement.Conclusion Color Doppler ultrasonography and color Doppler flow imaging can provide very reliable informations for definitely diagnosing the prenatal fetal umbilical cord round the neck.
