旋转DSA技术在腹部介入中的应用

摘　　要：目的 :探讨旋转DSA技术在腹部介入中的应用 (主要为肝癌患者的造影与治疗 )。方法 :应用Toshiba 2 0 0 0数字减影血管造影系统对疑有血管纡曲病变的 5 5例腹部介入患者先进行常规正位血管造影 ,然后选择合适的技术参数进行旋转DSA检查。结果 :旋转DSA检查中 5 0例 ( 91%)造影图像均避开了多支血管相互的重叠 ,清晰地显示相关血管的来源和走向。 5例快速旋转DSA检查多角度图像虽清晰显示出供血动脉 ,但因其局部血管过度纡曲上下重叠 ,局部血管走向显示欠佳。结论 :旋转DSA技术能够更好地显示腹部病变的供血动脉和分支结构 ,多方位、清晰地显示纡曲动脉分支的走行、正常解剖和异常改变 ,是对常规血管造影的重要补充 。Objective:To discuss the application of rotational DSA technique in the abdominal intervention (mainly the angiography and treatment of the hepatic carcinoma).Methods:Conventional anterior posterior projection angiography of the liver was performed on Toshiba 2000 DSA system in 55 cases with suspected overlapping and tortuosity of the vessels, subsequently rotationary DSA was additionally performed with appropriate parameters.Results:The overlapping of the vessels was avoided in 50 of 55 cases (approximately 91%) on rotational DSA,and the origin and course of the vessels were clearly displayed.In the remaining 5 cases the feeding artery was clearly demonstrated but the whole course could not clearly displayed due to excessive tortuosity of the vessels.Conclusion:Rotational DSA can clearly display the feeding arteries and the branches of overlapped and tortuous vessels from different projections. Thus it is an important complement of routine angiography and can provide reliable diagnosis for the treatment of the hepatic carcinoma.
