摘　　要：文章详细介绍了我院利用智慧医学平台实施慢性病信息化管理的探索性实践,以及该实践在提高患者健康指标上取得的初步成效。认为信息化建设中,对患者主体特点和需求进行分析、立足于前沿循证医学证据和理论,建立高标准的患者信息使用规范非常必要。The paper introduces the exploratory practice of the hospital in applying smarter health platform to the information-based management of chronic disease and the preliminary effect obtained by the practice on the improvement of patients' health indicators. It is believed that during informatization, it is very necessary to analyze the characteristics and demands of patients as the subject, base on the evidence and theory of cutting-edge evidence-based medicine, and establish high-level norms for patient information use.
