冠心病与血清C-反应蛋白浓度的关系及调脂药物干预治疗的前瞻性研究  被引量:4

摘  要:目的 探讨血清高敏C-反应蛋白(hs-CRP)的浓度对冠心病严重程度的影响及评价他汀类降脂类药氯伐他汀的某些降脂作用。方法 采用乳胶增强免疫(超敏)比浊法,定量检测健康对照组120名、稳定性心绞痛(SA)72例、不稳定性心绞痛(UA)80例和急性心肌梗死(AMI)52例患者治疗前血清hs-CRP及其它生化指标。对血清hs-CRP增高的患者,随机分为氯伐他汀治疗组、常规治疗组,分别在用药后4w、8 w检测上述指标。结果 ①AMI组、UA组治疗前血清hs-CRP浓度、Fbg浓度,明显高于SA组及健康对照组(P<0.01);AMI组的血hs-CRP浓度、Fbg浓度,明显高于UA组(P<0.01);而SA组血hs-CRP浓度、Fbg浓度与正常组比较,无明显差异(P>0.05)。②治疗前AMI组、UA组、SA组,血清hs-CRP增高(均大于3 mg/L)的特异性分别为92.3%,72.5%,10.5%。③氯伐他汀治疗组经治疗4 w、8 w较常规治疗组血hs-CRP明显降低(P<0.01)。④治疗前AMI组Lpa值明显高于UA组、SA组和正常组(P<0.05);而UA组、SA组与正常组之间Lpa值均无明显差异(P>0.05)。结论 ①炎症反应在急性冠脉综合征中起着一定作用,hs-CRP可能是致冠心病独立的危险因素之一。②血hs-CRP及Fpg的联合检测,可提高对冠心病患者发生急性冠脉综合征危险性的预测价值。③血Lpa可能是发生急性冠脉综合征独立的危险因?Objective To explore the effects of serum concentration of high-sensitivity C-reactive protein(hs-CRP) on coro-nary heart disease and evaluate certain functions of Fluvastain in reducing lipid. Methods The particle enhanced immnuo-turbidimetric had been used on 4 groups-control group 120 persons.stable angina pectoris(SA) group 72 patients,unstable angina pectoris(UA) group 80 patients and acute myocardial infarction(AMI) group 52 patients,to analyze the serum con-centrations of their hs-CRP and other biochemical indexes respectively fast before treatment. Those with higher serum con-centrations had been randomly categorized into two groups,one group had been treated with Fluvastain and the other had treated regularly for 8 weeks,the indexes were respectively determined after 4 weeks and 8 weeks. Results ①The serum concentrations of hs-CRP and plasma Fbg quantities in AMI and UA groups before treatment were significantly higher than those in SA group and control group(P<0. 01),and those in AMI group were significantly higher than those in UA group. While comparing those in SA group with the control group,there was no statistical difference (P>0. 05). ②Before treatment,the speciality of serum hs-CRP concentration in AMI,UA and SA was 92. 3%,72. 5%and 10. 5% respectively.③After 4 or 8 weeks,the serum hs-CRP concentrations of those in Fluvastain treatment group were notably decreased, compared with those in the regular treatment group(P<0. 01). ④Before treatment,serum Lpa Ievel in AMI group was significantly higher than that in UA,SA and control groups,while among UA,SA and control groups,there was no signifi-cant difference(P>0. 05). Conclusion ①Inflammation plays a key role in the development of acute coronary syndrome. And hs-CRP is an independent risk factor on coronary heart disease. ②The serum hs-CRP concentration and plasma Fbg quantity can predict the risk of the development of acute coronary syndrome in the patients with coronary heart disease. ③Serum Lpa concentration may be an independent ri
