主-肺动脉间隔缺损的二维和彩色多普勒血流图表现特征

摘　　要：目的 :探讨二维超声心动图 (2 DE)和彩色多普勒血流图 (CDFI)对主 -肺动脉间隔缺损心内畸形的诊断价值 ,并讨论超声心动图表现特征。方法 :自 1 996年 6月至 2 0 0 2年 2月应用彩色多普勒超声的二维图像 ,多普勒频谱和彩色血流显像对 3例主 -肺动脉间隔缺损的解剖及血流动力学异常进行观察 ,将其声像图表现与手术结果进行对比分析 ,并结合文献总结其 CDFI的诊断与鉴别诊断。结果 :彩色多普勒超声特征是 :二维超声心动图多切面显示主 -肺动脉间隔缺损 ,主肺动脉内异常高速血流由升主动脉射入 ,主肺动脉扩张 ,左、右肺动脉增宽 ,左心增大。其中 型 2例 , 型 1例 ,合并室缺 1例。彩超诊断与外科手术结果相符。结论 :2 DE和 CDFI可对主 -肺动脉间隔缺损提供正确的诊断和分型及发现合并心内畸形 ,具有无创、安全、准确及可重复应用等优点 ,是主Objective:To study the value of two dimensional echocardiography(2DE) and color Doppler flow imaging(CDFI)for diagnosis and to discuss the features of cardiac malformation of aortopulmonary septal defect(APSD) Methods:Three cases with APSD from June 1996 to February 2002 were reported The diagnosis was confirmed by 2DE,CDFI and Doppler spectra The echocardiographic characteristics were compared with the operative results The diagnosis and differential diagnosis of APSD in CDFI were summarized in literature review Results:The echocardiographic characteristics were:aortopulmonary septal defect;the turbulent shunt flow shown from ascending aorta into the pulmonary artery;main pulmonary artery,right and left pulmonary artery and left ventricle were dilated According to two dimensional ultrasound,APSD could be classified into 3 types:Ⅰ?Ⅱ and Ⅲ(Ⅰ,two cases,one of them complicated with ventricular septal defect,VSD;Ⅲ,one case) The echocardiographic charactwristics of APSD were correspondant to the surgical results Conclusions:2DE and CDFI can provide accurate classification and diagnosis of APSD with or without intracardiac malformations Both of them are noninvensive,safe,accurate and repeatable methods which are the first choice for diagnosis of APSD
