摘　　要：目的　探讨 10 %高渗氯化钠在治疗充血性心力衰竭 (CHF)伴有低钠血症时的安全性和有效性。方法　选择 110例CHF伴有低钠血症的患者 ,在常规综合治疗 (休息、吸氧、强心、利尿、扩血管 )及治疗原发病等综合治疗的基础上 ,对治疗组 6 0例和对照组 5 0例进行对比分析。治疗组在限水的同时给予 10 %氯化钠治疗。对照组限水但不补钠。观察治疗前后两组血钠浓度、血钠与心功能改善时间。结果　治疗组和对照组治疗前血钠浓度分别为 (12 7.13± 4 .99)mmol/L、(12 6 .0 2± 5 .98)mmol/L(P >0 .0 5 ) ;治疗后分别为(137.38± 4 .5 4 )mmol/L、(12 4 .36± 8.72 )mmol/L(P <0 .0 1)。治疗组和对照组治疗后血钠改善时间分别为 (3.72± 2 .0 5 )天与 (10 .72± 5 .2 8)天 (P <0 .0 1)。心功能改善时间分别为 (6 .4 0± 3.10 )天、(10 .5 8± 4 .2 1)天 (P <0 .0 1)。结论　 10 %氯化钠在CHF低钠血症时的应用安全有效 ,可使低钠血症和心功能在短期内得到改善。Objective To explore the safety and efficacy of 10% sodium chloride in treating congestive heart failure(CHD) complicated with hyponatremia. Methods Routine and comprehensive treatment for primary disease was performed in 110 cases of CHF complicated with hyponatremia, on the basis of which 60 cases in treatment group and 50 cases of control group were comparatively analyzed. The patients in treatment group were treated with 10% sodium chloride who were limited to water at the same time, while the patients in control group were not given sodium chloride but limited to water. Serum sodium level,the time for improvement of serum sodium and cardiac function were observed in two groups before and after treatment. Results Serum sodium concentrations of 10% sodium chloride solution in treatment group and control group were (127.13±4.99) mmol/L, (126.02±5.98) mmol/L(P>0.05) and (137.38±4.54) mmol/L, (124.36±8.72 )mmol/L(P<0.01), respectively before and after treatment. The times for improvement of serum sodium in treatment and control group were ( 3.72 ±2.05)days and (10.72±5.28) days(P<0.01) , respectively,and the times of cardiac function improvement were (6.40±3.10)days and (10.58±4.21) days(P<0.01),respectively. Conclusion 10% sodium chloride solution is safe and effective in treating CHF complicated with hyponatremia, which can improve the hyponatremia and cardiac function in a short period.
