摘  要:目的 了解肺炎链球菌对青霉素和其它抗生素的耐药情况 ;对 40株青霉素敏感菌株和 30株青霉素不敏感菌株进行pbp2b基因扩增产物RFLP图谱相关性研究。方法 采用肉汤稀释法、E 试验和K B纸片法进行抗生素药物敏感试验 ;通过用特异性引物pF和pR对肺炎链球菌进行PCR扩增 ,并对扩增产物进行限制性内切酶HaeⅢ消化反应。结果 肺炎链球菌对青霉素的不敏感率为9.9%~ 14.6 %;40株青霉素敏感菌株 (MICs≤ 0 .0 94μg ml)中的 39株 ( 97.5 %)为s1、s2和s3三种之一 ,1株为r1;30株青霉素不敏感菌株 (MICs≥ 0 .12 μg ml)中的 2 6株 ( 86 .7%)为r1~r6六种之一 ,其余 4株为s1。结论 青霉素和 β 内酰胺酶类抗生素对北京地区的肺炎链球菌具有活性 ;肺炎链球菌的耐药性和pbp2b基因扩增产物图谱之间有相关性。Objective To investigate the prevalence of penicillin and other antibiotics nonsusceptibility (including intermediate and resistant) among Pneumococci and to assess the correlation of pbp2b amplicon profiles to penicillin resistance in pneumococcal isolates by PCR and RFLP analysis. Method Susceptibility to antibiotics was determined by broth microdilution, E-test, and K-B disk. Primers pF and pR were used to amplify the pbp2b gene and its amplicon was subjected to HaeⅢ digestion. Results The percentage of nonsusceptilibity of S.pneumoniae to penicillin G raised from 9.9% in 1997 to 14.6% in 2000; pbp2b amplicon of 97.5% (39/40) pneumococcal isolates for which MICs of penicillin were ≤0.094μg/ml had one of three different susceptible restriction profiles, and 86.7% (26/30) isolates for which MICs were ≥ 0.12μg/ml had one of six. Low-concentration penicillin resistance (MICs=0.12μg/ml to 0.5μg/ml) was associated with sensitive HaeⅢ profiles (s1) in 4 isolates. Conclusion Penicillin and beta-lactams retained their activity against S.pneumoniae in Beijing. Restriction fragment length polymorphism (RFLP) analysis of pneumococcal pbp2b-specific amplicons is feasible for penicillin resistance screening. [
